摘要
目的 探讨烧伤后血浆IL 8水平的变化与烧伤严重程度及延迟复苏的关系。方法 对 2 4例烧伤总体表面积 (TBSA)为 30 %~ 92 %的烧伤患者 ,按面积和深度分为四组 ,分别于伤后 1 8h,1、2、4、7d取血 ,分离血浆 ,采用放射免疫法测定IL - 8的水平。结果 与正常组比较 ,TBSA <50 %组血浆IL - 8水平在伤后 1 8h即显著升高 (P <0 .0 1 ) ,后呈下降趋势 ,至伤后 7d仍高于正常 (P >0 .0 5) ;TBSA >50 %组IL - 8水平在伤后各时相点明显高于TBSA <50 %组 (P <0 .0 1或P >0 .0 5) ;Ⅲ度TBSA >30 %组IL - 8水平在伤后各时相点明显高于Ⅲ度TBSA <30 %组 (P <0 .0 1或P <0 .0 5)。延迟复苏组IL - 8水平在伤后各时相点明显高于早期复苏组 (P <0 .0 1或P <0 .0 5) ,而且下降趋势平缓。结论 烧伤后和烧伤后延迟复苏均能诱导机体产生大量IL - 8。血浆IL - 8水平与烧伤面积大小。
Objective To observe the alterations of plasma levels of IL-8 in different burn area patients and its role in burn with early fluid resuscitation or delayed resuscitation. Methods 24 patients were divided into four groups according to burn area and depth. The plasma was separated from blood samples of 24 patients at post burn (PB) 18 h,1,2,4,7 d.The Il-8 in plasma was detected by RIA (radioimmunoassay).Results The plasma level of IL-8 in TBSA< 50% group increased significantly at PB 18 h(P<0.01),Persisting to PB 7 d(P<0.05);The IL-8 level of TBSA>50% group was significantly higher than that of TBSA<50% group(P<0.01 or P<0.05);The IL-8 level of III degree TBSA>30% group was also signifcantly higher than that of III degree TBSA<30% group (P<0.01 or P<0.05);The plasma level of IL-8 in delayed resuscitation group was significantly higher than that in early fluid resuscitation( P<0.01 orP<0.05).Conclusion Burn injury could result in production of IL-8,and the alteration of plasma IL-8 was influenced by severity of burn and burn with delayed resusctiation.
