摘要
目的 :探讨硅油进入前房后对房角组织的影响。方法 :将 2 5只健康新西兰白兔随机分成实验组和对照组 ,分别于前房注射硅油和平衡盐溶液 (BSS)注射后据眼压情况再次分组 ,并于注射后第 3 0日将眼压增高兔眼中的硅油取出。分别于注射后第 5、 15、 3 0、 60、 90日采集标本行透射电镜观察。结果 :2 1/ 5 0的兔眼注射硅油后出现眼压增高。电镜下可见小梁间隙增宽 ;胶原及弹性纤维增生 ,内皮网内不定形物堆集并随高眼压持续时间延长而更趋明显。硅油取出后眼压和房角组织逐渐恢复正常。眼压正常眼初期可见小梁网轻度扩张 ,房水丛管腔壁内皮细胞吞噬囊泡增多 ,后逐渐消退。结论 :硅油进入前房后可引起持续性眼压增高并由此引起房角组织发生病理改变 ,硅油取出后可恢复正常。
Objective:To study the affects of silicone oil (SO) injected into anterior chamber on filtering angle tissues. Methods:24 rabbits were randomly divided into two groups.The experimental group included 50 eyes receiving SO which was injected into anterior chamber.The control group included 10 eyes,one of which was kept as normal control,the others were injected with balanced salt solution (BSS).According to the intraocular pressure (IOP) was normal or elevated,the experimental group was further divided into group A and B.The silicone oil in the eyes of group B were removed on 30 days after injection.The specimen of filtering angle tissues were collected and then examined under transmission electron microscope on 5,15,30,60,90 days after injection,respectively. Results:Elevation of IOP was occurred in 21 out of 50 eyes after receiving SO.During the early period,in group A,the trabecular meshwork dilated slightly,the vacuoles increased in endothelial cells of inner wall of the aqueous plexus canal.No marked abnormality was found in the later specimen.In group B,the space trabecular broadened,there were proliferation of collagenous and elastic fiber and accumulation of extracellular plague materials.The changes mentioned above were more remarkable with the prolongation of the duration of the intraocular hypertension.After the removal of SO,the intraocular pressure and the ultrastructure of angle tissues were gradually recovered. Conclusions:Silicone oil injected into anterior chamber may cause sustained elevation of IOP and corresponding pathological change of filtering angle,but the phenomenon would recover after removal of SO.Silicone oil is not toxic or destructive to the tissues of anterior chamber angle.
