摘要 目的 探讨米非司酮对人脐静脉超微结构的影响。方法 将中孕引产病例随机分为米非司酮组 (n =7)与对照组 (n =5 )。米非司酮组在水囊引产前 6h服用米非司酮 1 5 0mg ,对照组只行水囊引产术。胎盘娩出后 ,即取脐静脉制备透射电镜样品并观察。结果 与对照组相比 ,米非司酮组的脐静脉可见下列形态改变 :( 1 )内皮细胞胞浆呈空泡状 ,甚至发生断裂 ,粗面内质网囊泡化 ;( 2 )平滑肌细胞核染色质浓缩与边聚 ,核周腔扩大 ;线粒体空泡化 ;胞浆中糖原颗粒缺如或减少 ;( 3)间质水肿。结论 米非司酮可造成中期妊娠脐静脉的缺氧性损伤。 Objective To investigate the influence of mifepristone on the ultrastructure of fetal umbilical vein. Methods Twelve pregnant women with indication for pregnancy termination were recruited and randomly divided into 2 groups. In mifepristone group( n =7),150mg mifepristone per os was given six hours before labor was induced. Labor induction was performed by extra amniotic insertion of waterbag in the control group( n =5). Immediately after delivery of fetus,umbilical vein was acquired and prepared for transmission electron microscopic examination. Results In comparison with the control groups,the umbilical vein of mifepristone group displayed the following morphological changes:(1)endothelial cell,the cytoplasm was vacuolated,even broken;in the rough endoplasmic reticulum,vacuolization was found. (2)vascular smooth muscle cell,the chromatin was condensed and marginated and perinuclear space was dilated;mitochondria were vacuolated;glycogen granules in cytoplasm were reduced in number or disappeared. (3)the interstitial tissue was oedema. Conclusion Mifepristone can cause a hypoxic damage in the second trimester fetal umbilical vein.
