摘要 目的 探讨我国成人不同 BMI水平对血糖、血脂及血压的影响。方法 对 1994年全国糖尿病协作组经OGTT(75 g葡萄糖 )证实的 2 5岁以上的糖耐量正常和异常人群 15 6 18例的资料进行统计分析 ,按体重指数 (BMI)不同分成 BMI<2 1、2 1~、2 3~、2 5~、2 7~及≥ 30共 6组 (即 G1~ G6 ) ,糖尿病按 1985年 WHO糖尿病诊断标准。血清甘油三酯≥ 2 0 0 mg/ dl和胆固醇≥ 2 30 mg/ dl为判定高脂血症的条件 ,血压≥ 16 0 / 90 mm Hg为高血压的判断标准。结果 随 BMI逐渐升高 ,G1~ G6空腹及 OGTT2小时血糖 (FPG、PG2 h)、血浆胰岛素水平 (INS)、血压 (DBP、SDP)及血甘油三酯水平(FTG)均逐渐增高 ,组间差异有显著性。 BMI2 5~ 2 7组 ,FPG已达 112 .9± 1.0 m g/ dl,PG2 h达 15 3.5± 1.3mg/ dl,但平均血压及 FTG尚在正常范围。BMI2 7~ 30组 ,FPG已达 116 .7± 0 .8mg/ dl,PG2 h达 16 0 .3± 1.6 mg/ dl,FTG达 146 .9±1.0 mg/ dl,血压均值达 134 .4/ 84.8mm Hg。BMI≥ 30组则更高。经 L ogistic逐步回归分析 ,分别以是否发生糖尿病、高脂血症和高血压为因变量 ,以年龄、性别、血清胰岛素水平、平均血压、血清 TC、TG及 BMI的分组变量为自变量 (分析高血压时自变量中不包括平均血压 ,分析高脂血症时自变量不包? Objective To explore the effect of different BMI levels in adults on blood glucose, lipid and blood pressure.Method Clinical data of 15618 subjects aging over 25 years, who received OGTT (75g glucose), were analyzed in National Diabetes Mellitus Collaborative Study Groups in 1994. They were divided into six groups according to BMI level, which were BMI<21,21~,23~,25~,27~ and ≥30(G1~G6). The 1985 WHO criteria for DM were used. Hyperlipidemia criteria were defined as TG≥200mg/dl and CHO≥230mg/dl. Hypertention criteria were BP≥160/90mmHg.Results With BMI increasing from G1 to G6, the level of fasting plasma glucose(FPG), 2h plasma glucose after OGTT (PG2h), plasma insulin, blood pressure (BP), and TG showed a trend of linear increase, and there was significant difference among the groups. The mean FPG and PG2h in group with BMI 25~27 reached 112 9±0 7mg/dl and 153 5±1 3mg/dl respectively, but BP and TG were still in normal range. In group with BMI 27~30, FPG and PG2h were 116 7±0 8mg/dl and 160 3±1 6mg/dl respectively, the mean fasting TG was 146 9±1 0mg/dl,the mean BP was 134 4±88 8mmHg. In group with BMI≥30, the above value was even higher. Logistic regression analysis showed that BMI in the group of 25~27,27~30 and ≥30, was positively associated with the development of DM, hypertriglyceride and hypertention, and OR value ranged from 1 6 to 6 56. Conclusion When BMI was over 25, the risk of the development of DM, hypertention and hyperlipidemia was significantly increased.
