拓朴替康治疗小细胞肺癌近期疗效观察

The Effect of Topotecan in the Treatment for Small Cell Lung Cancer

[目的]观察拓朴替康治疗小细胞肺癌的疗效和毒副反应。[方法]拓朴替康单药治疗剂量为1.2mg/m2,联合剂量为1mg/m2,静脉输注30分钟 ,连用5天 ,联合卡铂剂量为350mg/m2,21天为一周期 ,连用2个周期评价疗效。[结果]17例中CR1例 ,PR12例 ,有效率76.5%。初治的有效率为90 %,复治的有效率为57.1%。3例脑转移的患者 ,2例PR。毒副反应主要为中性粒细胞及血小板减少 ,18例中17例有不同程度的中性粒细胞减少 ,其中达Ⅲ、Ⅳ度的13例 ,血小板达Ⅲ、Ⅳ度8例。[结论]拓朴替康单药治疗复发小细胞肺癌疗效确切 ,联合卡铂治疗可以耐受 ,疗效增加。

To observe the effect and toxicity of topotecan in the treatment for small cell lung cancer(SCLC).Topotecan alone was used to treat patients at a dose of 1.2mg/m2,and combined with CBP 350mg/m2 at a dose of 1.0mg/m2,daily 30 min iv infusion for 5 days.Treatment was repeated once every 3 weeks.At last 2 cycles were given.In 17 SCLC patients,there were 1 CR,12 PR,with an overall response rate of 76.5%.The response rate of patients with or without prior chemotherapy was 90% and 57.1%,respectively.In 3 patients with brain metastasis,there were 2 PR.The major toxicity were neutropenia and thrombocytopenia,which occurred in 13 and 8 cases,respectively.[Conclusion]Topotecan is an effective agent for single use in the treatment for recurrent SCLC,and it combined with CBP is tolerable and effective.The major toxicity is neutropenia and thrombocytopenia.
