摘要
目的 :评价慢性阻塞性肺部疾病 (COPD)肺部密度和体积在普通CT双相扫描下的表现。方法 :对 30例COPD患者与 5 0例健康成人 ,使用普通CT分别进行了深吸气末和深呼气末两个时相的扫描 ,在全肺选上、中、下三个区域 ,测量、计算肺部CT值和肺野体积值 ,把两组结果作比较。结果 :COPD组在深吸气末的CT值与正常组无明显差别 ,分别为 (- 90 1± 34)HU和 (- 886± 2 2 )HU ,但在深呼气末时则明显低于正常组 ,分别为 (- 870± 36 )HU和 (- 6 99± 5 2 )HU ,呼、吸气之间的差值两组分别为 (30± 17)HU和(187± 4 7)HU。在呼气CT像上 ,COPD组肺野体积仅减少 17% ,小于正常组 (40 % ) ,提示COPD患者中大多数有气体贮留。结论 :COPD患者较正常人有明显的气体贮留 ,可用普通CT对肺部作双相扫描进行诊断。
Objective:To observe and evaluate the pulmonary density and volume in inspiratory and expiratory phase of COPD patients with ordinary CT scanners.Methods:Thirty COPD patients and 50 healthy people were performed by dual-phase scanning with ordinary CT scanners,three selected areas,the upper, middle and lower portion was measured to get the pulmonary attenuation (HU) and pulmonary volume in both groups.Results:The CT values during deep inspiration phase in COPD group was similar with that in control groups,( -901±34)HU and (-886±22)HU respectively. But it was lower than that in control group during deep expiration phase〔(-870±36)HU vs (-699±52)HU〕. The difference between two phases in two groups were 3017HU and 18747HU.The volume decreased only 17% in COPD group at expiratory phase which was less than in control group(40%).All showed that most COPD patients had air-trapping.Conclusion:Patients with COPD has more obviously air-trapping than normal ones, which can be detected by ordinary CT scanners.
