小切口复位带锁髓内针治疗股骨干骨折(附54例报告)

Femoral Shaft Fractures Treated by Interlocking Intramedullary Nail and Open Reduction with a Short Incision(A Report of 54 Cases)

目的 :对股骨干骨折采用小切口开放复位带锁髓内钉治疗进行回顾性分析。方法 :本组 5 4例 ,男 42例 ,女12例 ;年龄 19～ 62岁。闭合性骨折 46例 ,其中骨不连 6例 ,开放性骨折 8例。全部采用小切口复位带锁髓内钉内固定 ,静力固定的 ,术后 6～ 10周拍片示骨折端有少量骨痂形成即改动力固定 ,骨不连病例均动力固定 ,术后所有病例均早期负重。结果 :全部病例获得随访 ,随访时间 6～ 30个月。全部骨折骨性愈合 ,平均愈合时间 15周 ( 8～ 2 4周 )。无断钉 ,松动和感染情况发生。结论 :本方法具有适应症广 ,骨折愈合快 ,组织损伤小 ,固定牢固 ,复位可靠 ,功能恢复快 ,并发症少等优点 ,又不需C臂X线机和骨科牵引床 ,适合在基层医院临床应用。

Objective: 54 cases of femoral shaft fractures w ith complete records were reviewed.Methods:From July 1997 to February 2000,the p atients,42 male and 12 female,aged ranging from 19 to 62 years old.Clo se fracture were 46 cases and open fracture were 8 cases.All cases were treated by interlocking intramedullary nail and reduced with a short incision.Dynamic in terlocking replaced static one in 48 cases when a little callus can be seen in X -ray film(6～10 weeks postoperation).Dynamic interlocking in 6 cases were nonuni on.All patients were permitted to bear weight immediately as soon as he or she c ould be after operation.Results:54 cases were followed-up for 6 to 30 months.All fractures were bony union.The time for union averaged 15 weeks(8～24 weeks).No in fection,no screw broken and loosen.Conclusion:The method has the advantage of wi de indication;less complication;united quickly because of certain replace and st able fixation;less injury give rapid recovery.In addition,the C-arm X-ray machin e and orthopedic tractive bed are not necessary,it may be widely used in basic h ospital.
