掌背动脉复合组织瓣在手指复合损伤中的应用

The Clinical application of the dosal metacarpal artery composite flap

目的 用简单、有效、可靠的方法修复手指掌、背侧复合组织缺损。方法 根据掌背动脉的解剖特点 ,设计以第二、三掌背动脉为蒂的肌腱皮瓣、骨皮瓣、肌腱骨皮瓣逆行移位修复示、中、环指掌、背侧复合组织缺损共 16例。结果 14例皮瓣顺利成活 ,2例出现静脉危象 ,皮缘部分坏死 ,经拆线、换药后伤口愈合。术后 3周开始系统功能练习 ,9例术后随访 3个月～ 2年 6个月 ,皮瓣柔软有弹性 ,外形佳 ,感觉、运动功能恢复良好。结论 第二、三掌背动脉复合组织瓣血运可靠 ,并含有感觉神经 ,手术操作简单 ,用于修复示、中。

Objective To use simple and reliable method in restoring composite defect in the finger.Methods Composite tissue defect in the finger of 16 cases were treated by transfering the dosal metacapal artery composite flaps.Results The flaps survived in 14 cases and had partial necrosis in 2 cases,becaue of appearing vein reflux disturbance.The period of following up was 3 months～30 months,the flaps were satisfactory in appearance and function.Conclusion The dosal metacapal artery composite flap is an ideal flap for repairing composite tissus defect in the finger.
