摘要
目的 观察雾化诱导痰中嗜酸性细胞 (EOS)和嗜酸性细胞阳离子蛋白 (ECP)在过敏性哮喘诊断中的意义。方法 30例过敏性哮喘和 13例健康志愿者进行超声雾化诱导 ,挑取痰栓 ,进行嗜酸性细胞计数和分类并测定上清液嗜酸性细胞阳离子蛋白。结果 健康对照组中无嗜酸性细胞出现 ,在过敏性哮喘组有 6 6 .7%出现嗜酸性细胞 (P<0 .0 1) ;健康组 ECP为 138.6± 6 8.9ng/ml,哮喘组 ECP为 10 6 5 .2± 112 4.1ng/ ml,有显著差异性 (P<0 .0 1)。在过敏性哮喘组中对 17例患者 ,诱导痰中 EOS阳性率为70 .6 % ,外周血中嗜酸性细胞阳性率为 11.8% (P<0 .0 1)。结论 诱导痰方法在过敏性哮喘诊断中具有重要的参考价值 ,EOS在诱导痰中更易检出 ,比外周血更敏感 。
Objective To observe the diagnostic significance of eosinophil(EOS) and eosinophil cationic protein(ECP) of induced sputum on allergic asthma.Method The sputum of 30 allergic asthmatics and 13 healthy volunteers was induced by inhalation of 5% hypertonic saline solution.After the sputum samples were liquefied by 0.1% dithiothreitol (DTT),total EOS and differential cell counts were performed and ECP was determined in the supernatants.Results Compared with no detected EOS in the normal control group,EOS was found in 66.7% allergic asthmatics(P<0.01);allergic asthmatics had a higher level of sputum ECP than the normal control group(1065.2±1124.1ng/ml versus 138.6±68.9ng/ml,P<0.01).EOS was counted both in the samples of induced sputum and peripheral blood,the positive rates were 70.6% and 11.8% respectively (P<0.01).Conclusion Induced sputum is very important to the diagnosing of asthma,and EOS is more easily detected in induced sputum than in peripheral blood.
