多穴位电脉冲刺激的脑皮层功能区fMRI研究 被引量:39

摘要 目的 探讨多穴位电脉冲刺激时的脑功能区变化状况。资料与方法 选择治疗偏瘫患者常用的一组穴位 (肩骨禺、手三里、曲池、合谷 )对 12例健康右利手志愿者的右上肢进行电脉冲刺激 ,同时进行BOLDfMRI扫描 ,运用t检验统计学方法分析得出刺激状态与静息状态信号对比的脑功能图像 ,观察脑皮层相应功能区的激活状况。另外 ,随机选择其中 5例于不同时间段重复观察 ,评价兴奋区的可重复性。结果 针刺状态下 ,12例受试者的对侧初级躯体感觉区 (SM1)被激活 ,其中有 9例可观察到一侧或双侧辅助运动区 (SMA)兴奋、8例运动前区 (PMA)兴奋。激活区检测的可重复性 >80 %。结论 多穴位电脉冲刺激能特定性地激活相应的脑皮层兴奋区 。 Objective To study the status of brain activation under electro acupuncture stimulation of multiple acupoints with fMRI.Materials and Methods Twelve right handed healthy volunteers were studied under electro acupuncture stimulation of multiple acupoints. Electro acupuncture stimulation was conducted through a group of acupoints (Jianyu, Shousanli, Quchi and Hegu) of the right hand, which are routinely used in the clinical treatment of hemiplegia. Functional responses were established by students' group t test analysis. In addition, reproducibility of activation was evaluated in 5 randomly selected subjects with repeated examination.Results Under electro acupuncture stimulation, the contralateral primary somatosensory cortex (SM1) was activated in all 12 volunteers. Besides, Uni or bilateral activation of the supplementary motor area (SMA) was observed in 9 subjects, and activation of the premotor area (PMA) in 8. The reproducibility of the activation was over 80%.Conclusion Acupuncture stimulation of multiple acupoints can activate the corresponding brain cortices. The select of acupoints for acupuncturing should be based on the clinical symptoms.
