改良巩膜隧道式切口在白内障囊外摘除术中的临床应用

Clinical Application of Modified Sclera Tunnel Incision on Extracupsular Cataract Extraction

目的 :探讨改良巩膜隧道式切口对现代白内障囊外摘除术的影响。方法 :对 75眼改良巩膜隧道式切口及 5 9眼普通梯形切口的术中虹膜损伤情况、玻璃体脱出后人工晶状体植入情况、术后瞳孔上移情况、术后 3个月矫正视力、角膜散光等行对照研究。结果 :改良巩膜隧道式切口术中虹膜脱出及色素脱落少 ,玻璃体脱出后人工晶状体仍可植入 ,术后瞳孔上移少 ,角膜散光轻 ,术后 3个月矫正视力、角膜透明情况与普通梯形切口无明显差异。结论 :改良巩膜隧道式切口可有效地提高手术安全性 ,减少切口引起的并发症 ,值得推广使用。

Objective:To study effects of modified sclera tunnel incision on extracupsular cataract extraction. Methods:Tunnel incision of 75 eyes were compared with conventional ladder shape incision of 59 eyes. The injures of iris in the operation, IOL implantation after vitreous inburst, up shift on pupils, the corrected visual acuity and corneal astigmatism 3 months after operation of two type imcision were studied. Results:The iris prolapse, pigment fall in the operation, up shift of pupils and corneal satigmatism in or after operation of modified sclera tunnel incision were much decreased and mild. IOL can still be implanted after vitreous inburst. But the corrected ivsual acuity after operation of 3 months and corneal transparence had no significant differences between modified tunnel incision and conventional ladder shape incision. Conclusion:Modified sclera tunnel incision can effectively increase the security of operation and the complications caused by operation. It may be further extended in clinical applicantion.
