广州市居民肠易激综合征及功能性便秘的流行病学调查

The epidemiology of irritable bowel syndrome and functional constipation of Guangzhou residents

目的 探讨广州市居民肠易激综合征 (IBS)及功能性便秘的流行病学情况。方法 采用整群随机抽样法抽取广州市居民 2 892人进行问卷调查 ,同时问卷普通内科及消化专科门诊连续病例各 10 0 6及 495例。结果 (1)广州市居民IBS的患病率为 5 6 % ,普通内科门诊及消化专科门诊中IBS所占的比例分别为 10 1%、34 3%。IBS女性患病率显著高于男性 ,男女比例为 1:1 5 1(P =0 0 2 7)。年龄对IBS的患病率无显著影响。IBS患者的就诊率为 2 2 4% ,男女就诊率差异无显著性。IBS各亚型构成比为 :腹泻主导型占 6 3 2 % ,便秘主导型占 2 7 0 % ,其他型占 9 8%。 (2 )广州市居民功能性便秘的患病率为 3 0 %。普通内科门诊及消化专科门诊中所占的比例分别为 5 4%及 4 8%。男性患病率显著低于女性 ,男女比例为 1:1 77。随着年龄的增加 ,功能性便秘的患病率升高。社区人群中功能性便秘的就诊率为 17 6 % ,男女差异无显著性。

Objective To investigate the prevalence rates of irritable bowel syndrome (IBS) and functional constipation in Guangzhou City. Methods A structured history using a questionaire on gastrointestinal symptoms during the preceeding one year was obtained from random sampling 2 892 residents, 1 006 consecutively patients who visited general practioner and 495 consecutively patients who visited gastroenterologists. All the questionaires were filled by trained doctors.Results The overall prevalence rates of irritable bowel syndrome and functional constipation in Guangzhou City were 5.6% and 3.0%. There were 10.1%?34.3% and 5.4%?4.8% visitors who complained IBS and functional constipation in the general internal outpatient department and gastrointestinal outpatient department. The female was predominat in both IBS and functional constipation. The ratio of male to female was 1:1.51 and 1: 1.77. With the increase of age, the prevalence rate of functional constipation rose; but IBS did not. There were 23.9% IBS patients and 17.6% functional constipation patients who had ever seen a doctor for these diseases in the community. The percentages of IBS subclasses were as the following: diarrhea predominant: 63.2%; constipation predominant: 27.0%; others: 9.8%.Conclusion The prevalence rates of IBS and functional constipation in Guangzhou City were lower than that in west countries.
