角膜缘干细胞移植术与结膜瓣移植术治疗翼状胬肉疗效比较 被引量:19

Comparison of the transplantation of corneal limbal stem cell with the transplantation of conjunctival flap for the treatment of pterygium

摘要 目的 :角膜缘干细胞移植术与结膜瓣移植术治疗翼状胬肉的疗效比较 ,寻找低复发率的治疗方法。方法 :6 8例 (73眼 )非复发性、复发性翼状胬肉及化学烧伤后的假性胬肉 ,分别进行胬肉切除加自体带蒂结膜瓣移植或角膜缘干细胞移植术 ,术后观察两组的泪膜破裂时间 BUT、角膜上皮修复时间 ,以及复发率。结果 :对非复发性胬肉 ,两组间各项指标比较无统计学差异 ;对复发性胬肉、化学烧伤后的假性胬肉 ,角膜上皮修复时间比较无显著差异 ,但角膜缘干细胞移植术的复发率明显低于结膜瓣组 ,BUT恢复快。结论 :对复发性胬肉及化学烧伤后的假性胬肉 。 Objective: To compare the transplantation of corneal limbal stem cells with the transplantation of conjunctival flap for the treatment of pterygium, and to evaluate the effectiveness and advantage of stem cells transplantation. Methods: Sixty-eight cases (73 eyes) were divided into two groups at random. Forty eyes were performed pterygium resection and conjunctival flap transplantation, 29 of which were non-recurrent, 10 eyes of which were recurrent, 1of which was pseudo-pterygium after chemical burns. Thirty-three eyes were performed pterygium resection and stem cells transplantation, 21 of which were non-recurrent, 10 of which were recurrent, 2 of which were pseudo-pterygium after chemical burns. The growth and epithelialization of the graft were observed with cornea fluorescent staining. The broken-up time (BUT) was measured. Results: There wais no statistically significant difference in non-recurrent cases between two groups. There was statistically significant difference of BUT and recurrent rate in recurrent cases between the two groups. However, there was no statistically difference in growth and epithelialization of the graft. Conclusions: Transplantation of corneal limbal stem cells has advantage over transplantation of conjunctival flap in treatment of pterygium especially the recurrent pterygium.
