残胃癌的临床特征及外科治疗(附29例分析)

Clinicopathological Characteristics and Surgical Treatment of Stump Gastric Cancer.

目的 : 分析残胃癌的临床病理特征并评价手术治疗残胃癌的效果。方法 : 回顾性分析 1980年 1月～ 2 0 0 0年6月本院外科收治的 2 9例残胃癌病例资料。结果 : 本组病例中 ,Ⅱ期 4例 ,Ⅲ期 10例 ,Ⅳ期 15例 ;其中 6例 (2 0 .6 %)施行姑息性手术 ,17例 (5 8.6 %)施行根治性手术 ,6例因肿瘤广泛转移而未行手术。手术死亡率 8.7%(n =2 ) ,根治性手术后 1年、3年和 5年生存率分别为 75 %、5 5 .6 %和 41.7%。结论 : 根治性手术是治疗残胃癌的有效方法。

Objective: To elucidate the clinicopathological characteristics and review the results of surgical treatment in 29 patients with stump gastric cancer in a single institute. Methods: The clinical data of 29 cases admitted during the period from 1980 to 2000 were restrospectively analyzed.Results: In this group of cases, pathological stage Ⅱ,Ⅲ and Ⅳ was 4, 10 and 15 cases respectively. Among them, palliative surgery was performed in 6 cases(20.6%), radical dissection in 17(58.6%) and 6 cases lost the chance of resection due to extensive dissemination. The mortality of surgery was 8.7%(n=2) while the 1-,3-,5-year survival rate in those who underwent radical dissection was 75%, 55.6% and 41.7% respectively. Conclusions: Radical dissection is an effective therapeutic procedure for stump gastric cancer.
