SLE并发播散性皮肤非典型分枝杆菌感染1例

A case of systemic lupus erythematos us complicated by disseminated cutaneous atypical mycobacteria infection

报告1例SLE患者并发孢子丝菌病样播散性非典型分枝杆菌原发皮肤感染。患者女性,33岁,水产经营者,既往有SLE病史15年。因“全身散发皮下红斑结节、破溃化脓伴轻痛2个月”收住院。皮损散在分布于四肢和躯干,为大小不等的硬性红斑结节,中央破溃、脓性分泌物和黄色结痂。皮损组织液涂片找到抗酸杆菌。组织病理示真皮肉芽肿性炎症。用阿米卡星霉素和复方磺胺甲口恶唑(SMZco)治疗1个月,大部分皮损消退好转。

This article reports a case of primary cutaneous atypical mycobacteriosis in systemic lupus erythematosus .A 33-year-old woman who dealt with aquatic product was admitted to our hospital because of sporadic erythema ,nodules,erosion and pyogenesis with mild pain about two months,and she had the history of lupus erythematosus about fifteen years.The lesions were erythema and hard nodules with different size which di stributed in extremities and trunk.Lesions became pyogenic and formate d yellow crust on surface soon.Acid-fast bacillus was found o n the smears of the lesions.The histo pathological changes showed the gla nulomatous inflammation in dermis.The majority of lesions we re disappeared or improved in the tre atment with amikacin and SMZco about one month.
