乙型肝炎病毒基因C区及其调控基因变异的研究进展 被引量:3

摘要 绝大多数乙型肝炎病毒感染始于儿童时期,可出现多种疾病过程,病毒可能被清除而痊愈,也可能在体内长期存在,随着疾病的逐渐演变,到成人期可发展为肝硬化、肝癌。儿童乙型肝炎病毒(HBV)感染者HBV基困C区及其调控基因变异的研究对明确与疾病过程的关系有十分重要的意义。儿童HBV感染前 C区变异中 1896 G→A最常见,其次为 1899G→A,其发生与 HBeAg血清转换有关,而且非HBV感染母亲的患儿高于感染母亲的患儿。C基因变异较少发生,在儿童慢性肝炎进展中不扮重要角色。调节基因变异多发生于BCP区域,最常见的为1762A→T/1764G→A的联合变异,该变异与慢性肝炎的活动有关。HBeAb阳性的HBV感染母亲所生新生儿易发生暴发性肝炎,与母亲体内的前C区变异株或/和BCP变异株垂直传播给其子女有关。
