摘要
目的本文用国产白细胞过滤器滤除浓缩血小板制剂中的白细胞 (PL型滤器 ) ,用于防止反复输注和血小板的病人发生血小板输注无效。方法从不同方法制备的浓缩血小板制剂中滤除白细胞 ,观察PL型白细胞过滤器对血小板回收率、白细胞去除率的影响。观察病人输注浓缩血小板制剂后 ,1小时和 2 4小时血小板上升值判定临床输注无效发生率。结果 70付PL型滤器过滤浓缩血小板 ,白细胞去除率 99 9% ,血小板回收率超过 90 %。 47例有多次输血史的血液病患者 ,每个病人输注手工制备的血小板至少 1次 ,输注机采血小板 1~ 3次。过滤组 2 7例病人 ,输注 70例次滤除白细胞的浓缩血小板制剂。对照组 2 0例病人 ,输注 5 0例次含有白细胞的浓缩血小板制剂。结果过滤组有 4/ 2 7例 (占 15 % ) ;对照组有 10 / 2 0例 (占 5 0 % )发生血小板输注无效 (P <0 0 5 )。
Objective Using domestic PL-filter to deplete leucocytes in the preparation of platelet-concentrate. To prevent inefficacy in platelet transfusion in cases of repeated platelet transfusion. Methods PL-filter was used to filter off leucocytes from platelet-concentrates prepared by means of different methods. Recovery rate of platelet and degree of leucocyte depletion were observed. Increase in platelet count in patients 1 hr.and 24 hr.after platelet transfusion were determined.Results 70 sets of PL-filter were used to prepare platelet concentrate. The recovery rate of platelet was over 90%, with a leucocyte-depletion of 99.9%. Patients were divided into 3 groups: 1) 47 patients with hemopathic diseases have received multiple blood transfusion. At least one transfusion of manual prepared platelet and 1-3 transfusions of machine prepared platelet. 2) Filter treated platelet group:27 patients 70 transfusions of filter-treated leucocyte-depleted platelet concentrate were given. 3) Control group: 20 patients 50 transfusions with platelet concentrate containing leucocytes. The results revealed the occurrence of transfusion inefficacy were: 4/27(15%) in filter group; 10/20(50%) in control group ( P <0.05).Conclusion PL-filter treatment is an ideal method for preparation of leucocyte-depleted platelet concentrate.
