前S1蛋白在HBV指标中的重要意义

Significance of Pre S1 Protein as an Important HBV Serological Marker

前S1蛋白是HBV衣壳蛋白的一部分 ,存在于大分子表面抗原蛋白之中 ,它是乙肝早期诊断的敏感指标 ,也反映了乙肝病毒的复制和乙肝病情的活动。本文研究显示 ,1 62例非乙肝病人血清中前S1蛋白全部阴性 ;31 1例乙肝患者中 ,前S1蛋白阳性率为 38 9% ,其中急性乙肝 93 8% ,慢性活动性肝炎 51 2 % ,在乙肝二对半检测模式中 ,大三阳中该指标阳性率 87 1 % ,小三阳为 32 1 %。以HBVDNA检测为标准 ,前S1蛋白的灵敏度为 94 1 % ,假阴性 5 9% ,总符合率 93 9%。

Objective Pre S1 protein, which is a part of HBV encapsulated protein and exists in the macromolesular surface antigen protein, is a sensitive marker for early diagnosis of hepatitis B and a reflection of HBVreplication and activily of pathological condition of hepatitis B as well. In 162 patients without hepatitis B, serology test was negative for pre S1 proteinin all of them, and in 311 patients with hepatitis B of various types, the overall positive rate of pre S1 protein was 38 9% (93 8% for acute hepatitis B and 51 2% for chronic active hepatitis). In the serological test battory of hepatitis B, positive rate of pre S1 protein was 87 1% in patients with positive HbsAg, HbeAg, HbcAb markers, and 32 1% in those with only HbsAg, HbeAb, HbcAb markers showing positive. Taking positive HBV DNA as the dingnostic criterium, sensitivity of pre S1 protein was 94 1%, false negative rate was 5 9% and overall coincidence was 93 9%.
