摘要
角膜接触镜因为美容而被广大青年近视眼患者所采用,尤其是长戴型软接触镜更为倍受欢迎。随着配戴人数的增加,角膜并发症也逐年增多,其中的角膜溃疡最为严重。本文收集我院1984.3.1～1990.12.31接触镜角膜溃疡30例(30眼)进行临床分析,并探讨其发病机理及防治方法。资料及方法30例患者(30眼)均戴角膜接触镜时出现症状,通过临床、微生物学检查及治疗效果,最后诊断为微生物性角膜溃疡。所有患者均进行角膜坏死物微生物培养,方法为表面麻醉后取溃疡坏死物在血琼脂培养皿上划线接种培养48小时。常规治疗使用抗生素滴眼,严重者球结膜下注射。
Microbial corneal ulcers are the most serious complication of con-tact lens use.The authors analyzed thinty patients (30 eyes) with co-rneal ulcers associated with contact lens wear.Thirteen eyes wereculture positive.P.aeruginosa was cultured from seven eyes(53.8%).One of the pathogens was gentamicin-resistant.S.marcescens fromtwo eyes(15.4%).S.aureus,M.luteus,P.alcaligenes and Candida albicanseach from one eye(7.6%).All patients had corneal scars.Two patientswere blind in one eye,whose final visual acuities were 3.3 and 2.0.Therresults show that contact lenses use is increasingly important riskfactor for the development of corneal ulcers.P.aeruginosa is the mostcommon pathogen.The authors reviewed the literatures and discussedthe mechanism and methed of diagonsis and treatment.
