摘要
[目的]探讨恶性肿瘤特异性生长因子 (TSGF)、细胞角蛋白19片段 (CYFRA21 1)、糖链抗原 (CA50)、癌胚抗原 (CEA)测定和四项联检在恶性肿瘤诊断中的价值。[方法]采用生化或放免分析法等检测239例恶性肿瘤病人血清TSGF、CYFRA21 1、CA50、CEA含量 ,与对照组相比较 ,并分析结果。[结果]恶性肿瘤组的血清TSGF、CYFRA21 1、CA50、CEA含量明显高于对照组 ,差异有显著性 (P<0.05);TSGF、CYFRA21 1、CA50、CEA测定的阳性率分别为53.6 %、29.7 %、8.0 %、8.0% ,除CA50与CEA无差别外 ,其他各组间的差异有高度显著性 (P<0.001) ;四项联检的阳性率明显提高。[结论]联合检测血清TSGF、CYFRA21 1、CA50、CEA含量能提高恶性肿瘤诊断的阳性率 ,而TSGF和CYFRA21 1测定在肿瘤普查中具有一定的价值。
To explore the diagnostic significance of combined determination of serum tumor specific growth factor (TSGF),cytokeratin19 fregments(CYFRA21 1),carbohydrate antigen(CA50)?carcinoembryonic antigen(CEA),for diagnosis of malignant tumor. The levels of serum TSGF,CYFRA21 1,CA50,CEA were determined by biochemical or immunoradiometric assay in 239 patients with malignant tumor and in 36 persons without tumor,and the results were analyzed. The levels of serum TSGF,CYFRA21 1,CA50,CEA were significantly different in the group of malignant tumor compared with those in the controls (P<0.05),and the sensitivity of four kinds of determination,the positive rate of TSGF,CYFRA21 1,CA50,CEA were 53.6%, 29.7%, 8.0%,8.0% respectively. There were significant difference of TSGF,CYFRA21 1,CA50 or CEA (P<0.001),and no difference of CA50 and CEA (P>0.05) between malignant and control.The positive rate significantly increased when combined determination of serum TSGF,CYFRA21 1,CA50,CEA. [Conclusion] Combined determination of TSGF,CYFRA21 1,CA50,CEA can increase the positive rate of diagnosis for malignant tumor,and determination of TSGF and CYFRA21_1 is of value in tumor mass screening.
