右旋心及合并畸形的二维超声心动图图像特征及规律性研究

Study on imaging character and regularity of two dimensional echocardiography on dextroversion and deformity

目的 探讨右旋心及合并畸形二维超声心动图 ( 2DE)图像特征及其规律性。方法 应用 2DE检查 78例右旋心及合并心脏其他畸形患者 ,发现其图像特征及规律性 ,76例行心血管造影对照 ,6 0例经手术证实。结果 根据 2DE图像特征对 74例作出正确诊断 ,诊断准确率 94.9%。右旋心合并 8种先心病 ,其中矫正性大动脉转位多见 ,单心室、右心室双出口和双腔心少见 ,三尖瓣闭锁、解剖二尖瓣功能三尖瓣闭锁、法洛四联症和室间隔缺损罕见。结论 右旋心合并 8种先心病 ,2DE图像特征及规律性明显。应用 2DE可对右旋心及合并畸形作出正确诊断 ,但检查者必须熟练掌握本病解剖。在检查手法上与正常位心脏和镜向右位心均不同。

Objective To explore the image characteristics and regularity of two dimensional echocardiography (2DE) on dextroversion and deformity.Methods Seventy eight patients with dextroversion and deformity were examined by 2DE and the image characteristics and regularicty of 2DE were found.Seventy six cases with dextroversion and deformity underwent angiocardiography examination after 2DE.Sixty cases were proved by surgical operations.Results Seventy four cases were diagnosed correctly by the features of 2DE imaging.The diagnostic rate of accuracy was 94.9% .Dextroversion was complexed by deformity of 8 kinds congenital heart diseases,among which corrected transposition of the great arteries were common,single ventricle,double outlet right ventricle and cor biloculare were not as common as the above,and tricuspid atresia,anatomy mitral and fanction tricuspid atresia,tetralogy of fallot and ventrcluar septal defect were rare.Conclusions Dextroversion are usually deformity of 8 kinds congenital heart diseases.The image characteristics and regularity of 2DE on dextroversion and deformity are obvious.2DE can be used to diagnose correctly dextroversion and deformity,but the operators of 2DE must have a clear idea of the anatomy of the dextroversion .The exploring way of the disease is different from those of normal position heart and mirror dextroversion.
