摘要
喉移植的目的在于恢复全喉切除术后患者近似正常的呼吸、吞咽或发音功能 ,并取得最佳的美容效果。但在满足上述条件的同时 ,接受咽喉移植的患者又面临肿瘤复发的可能性增加、排斥反应的发生和长期使用免疫抑制剂所致的免疫功能低下及其诱发的疾病等问题。因为喉是非生命所必需的器官 ,只有当喉移植所获取的利益远远大于其风险时 。
The goals of the larngeal transplantation are the restoration of near normal respiration,phonation,satisfactory deglutition,and the best cosmesis without or with a minimal risk of aspiration.While attaining these benefits,the patient undergoing this procedure must face significant risks,such as a potentially increased probability of cancer recurrence,graft rejection,and the inherent morbidity of chronic immunosuppression.Because larynx is a nonvital organ,laryngeal transplantation is not justifiable or ethical unless the benefit to the patient of having a transplanted larynx is greater than the danger of rejection and the risks of an immunosuppression regimen in a situation that is not life threatening.
