晚期血吸虫病巨脾切除对免疫功能的影响

Effects of splenectomy on immune function of patients with advanced schistosomiasis

目的 探讨晚期血吸虫病巨脾切除对免疫功能的影响。方法 对 2 0例晚期血吸虫病巨脾病人切除前、术后两周、术后半年、术后一年分别抽取周围静脉血测IgG ,IgA ,IgM ,C3 ,C4 ,TNF ,sIL 2R ,CD3+ ,CD4+ ,CD8+ ,用F检验作出显著性测定。结果 IgA ,IgM ,C3 ,C4 ,TNF ,sIL 2R ,CD8+ 切脾前后差异不显著 (P >0 0 5 ) ,IgG ,CD3+ ,CD4+ ,CD4+ /CD8+ 切脾后升高 ,差异显著 (P <0 0 1～ 0 0 5 )。结论 晚期血吸虫病巨脾切除后对体液免疫和细胞免疫均无不良影响 ,因而治疗性脾切除是合理的。

Objective To investigate influence of splenectomy on immune function of patients with advanced schistosomiasis. Methods Before and 2 weeks after the splenectomy, levels of IgG, IgA, IgM, C 3, C 4, TNF, sIL 2R, CD4 + and CD4 +/CD8 + in peripheral venous blood were determined in 20 patients with advance schistosomiasis. The follow up was conducted for 6 months to 1 year. Results The levels of IgG, CD3 +, CD4 + and CD4 +/CD8 + after the operation were significantly higher than those before the operation (P<0 01-0 05). As for other indexes, there were no remarkable differences between the preoperative level and the postoperative one (P>0 05). Conclusions In patients with advanced schistosomiasis, splenectomy has no significant effects on humoral and cellular immunity. Therefore, it is reasonable to use this technique in treatment of advanced schistosomiasis.
