单桥运动对急性脑梗死病灶局部脑血流量及肢体功能恢复的影响 被引量:10

Effect of unilateral bridging activities on regional cerebral blood flow and the recovery of the affected limb function of cerebral infarction in the early stage post stroke

摘要 目的评价患侧单桥运动和下肢被动运动对急性脑梗死病灶局部脑血流量(regionalcerebralbloodflow,rCBF)的影响,并探讨其与功能恢复的相关性。方法32例脑梗死患者分为单桥运动组17例,被动运动组15例,在运动治疗前后用单光子发射计算机断层扫描(SPECT)检测rCBF及Fugl-Meyer(FMA)量表和Barthel指数(BI)评定,进行相关分析。结果两种运动后即刻rCBF值比运动前明显提高(P<0.01),其中,单桥运动组的增高值大于被动运动组(P<0.05),rCBF增加值与FMA和BI评分的增加值呈正相关(r分别为0.657,0.689)。结论单桥运动比被动运动能更好地改善急性脑梗死病灶rCBF,且这种改善与运动功能和日常生活活动能力的改善相关。 Objective To study the effectiveness of rCBF in the unilateral bridging activities vs.passive movement of the affected lower limb,and further to analysize the relationship of the changes of rC BF and the recovery of the motor function and the ability of daily living.Method32inpatients with acute cerebral in farction were assigned into two grou ps:the unilateral bridging exercis e(n=17)and the passive movement (n=15).The SPECT was applied to all patient s to identity the rCBF and the FMA and B I were assessed before and after treatment.Result The patients' rCBF was improved after these two kin ds of exercises(P<0.01).Positive correlation was found between the rCBF increase and t he changes of the FMA assessment and B I scores(r =0.657,0.689respectively).Conclusion The unilateral bridging exercise is superior to the passive movement in impr oving the rCBF of patients with the ce rebral infarction.The in-creasing of rCBF was also contribute d to the improvement of the motor function and ability of daily living.
