摘要
目的探讨急性脑卒中后营养状况的变化、营养状况对急性脑卒中康复的影响和影响卒中后营养状况恶化的可能原因。方法收集临床住院的急性脑卒中132例。动态观察测定神经功能缺损评分、营养学指标及延髓麻痹和感染性并发症持续的时间。应用统计学相关方差分析,寻找它们之间的相关性。结果(1)神经功能缺损恢复程度不仅与入院时的营养状况相关,而且与住院期间营养状况恶化的程度更为相关。(2)卒中发生后营养状况呈恶化趋势。(3)卒中后营养状况恶化的程度与吞咽困难和感染性并发症持续的时间长短相关。结论(1)卒中后患者营养状况呈恶化趋势,恶化程度与神经功能恢复程度呈负相关。(2)营养状况恶化的原因是营养成分摄入减少和机体卒中后的高代谢。
Objective To determine the change of nutriture following 4weeks of hospitatizatio n and the effet of nutriture on the reh a-bilitation in acute stroke.To investigate the reasons of the nutriture d eterioration following acute strok e.Methods The study included 132patients with and acute stroke of less than 24hours duration.Nutritional parameters were evaluated at admis sion and after 1,2,4weeks.Neurological deficit was evaluated by the Chinese stroke scale at admission and 4weeks after the acute stroke.Results(1)The nu-triture deteriorated after acute stroke.(2)The rehabilitation was not only asso ciated with the nutriture at admission but also with the degree of nutriture deterioration after acut e stroke.(3)The degree of nutriture deteriorate d was negatively relative to the dura tion of the swallowing ab-normality and the complicated infec tion after acute stroke.Conclusions The nutriture following acute strok e trended towards deterioration and the degree of it' s deterioration is negatively relative of the rehabilitation in acute stroke.The basic reasons of nutriture deterioration are the nutrient intake reducing and the hyp ercatabolism and visceral consumpt ion.
