带锁髓内钉在股骨胫骨干骨折临床应用中的若干问题

Problems in Clinical Application of the Interlocking Intramedullary Nail in Treatment of Shaft Fractures in the Femur and Tibia

目的 :探讨交锁髓内钉的适应证、优缺点、手术技巧及并发症的预防。方法 :回顾性地分析了 5 3例股骨及胫骨干骨折应用交锁髓内钉的临床资料 ,从以上各方面进行讨论。结果 :骨折全部愈合 ,平均愈合时间 6 .3个月 ,浅表及深部感染率 5 .4% ,骨髓炎 1例 ( 1.9% )。膝关节屈曲 <90°者 1例 ,踝关节背屈 <10°者 3例 ,无畸形愈合及内固定失败者。结论 :交锁髓内钉是良好的内固定方法 ,具有骨折固定牢固、愈合良好、关节功能好的优点 ,值得推广应用。

Objective:To study the indication and skills of interlocking intramedullary nail(IMN)and its advantages and disadvantages and the prevention of complications.Methods:To analyze the clinical data of 53 cases with IMN in the tibial and femoral fractures retrospectively;to discuss every aspects above mentioned respectively.Results:All the fractures are healed,the mean time of healing is 6.3 months,superficial and deep infection rate is 5.4%,1 case osteomylitis,no malunion and nail failures.Conclusion:The IMN is a good internal fixation method,having the advantage of firm fixation,good healing and good articular function.
