心肌梗死心功能不全患者冠状动脉介入术后心功能恢复的时间进程 被引量:6

摘要 目的 探讨心肌梗死心功能不全患者冠状动脉介入术后心功能恢复的时间进程。方法30例左室射血分数(LVEF)<50%的心肌梗死患者,分别于冠状动脉介入术前及术后1周、1个月、3个月检查二维超声心动图,观察室壁节段运动指数(WMSI)和LVEF的变化。结果 心功能改善组于冠状动脉介入术后1个月即出现WMSI和LVEF的改善,至术后3个月继续改善;在心功能改善组中,急性心肌梗死(AMI)患者术后1个月上述指标即有明显改善,而陈旧性心梗(OMI)于术后3个月方有明显改善。结论 有大量存活心肌的心肌梗死心功能不全患者,冠状动脉介入术后心功能恢复呈进行性经过;心肌梗死患者尽早行冠状动脉介入术能使患者心功能及早恢复。 Objective To assess the time course of functional recovery after coronary intervention in patients with myocardial infarction. Methods 30 patients with myocardial infarction (left ventricular ejection fraction, LVEF< 50% ) were investigated. Wall motion score index (WMSI) and LVEF were serially measured by two-dimensional echocardiography (2DE) pre-intervention and 1wk, 1mo, 3mo after coronary intervention respectively. Results WMSI and LVEF can be seen significantly improved 1mo after coronary intervention in patients with functional improvement, and progressively improved 3mo after intervention. WMSI and LVEF improved significantly 1mo after intervention in patients with AMI, while 3mo after intervention in OMI. Conclusion In patients with myocardial dysfunction with more viable myocardium after myocardial infarction, functional recovery is progressive after successful coronary intervention. In patients with myocardial infarction, heart function can recover quickly by early coronary intervention.
