2型糖尿病胰岛素抵抗与白细胞介素-6关系的研究 被引量:6

摘要 目的 研究白细胞介素 6基因启动子区 174G/C(鸟嘌呤 /胞嘧啶 )的多态性与血清白细胞介素 6 (IL 6 )含量及 2型糖尿病胰岛素抵抗的关系。方法 对 196例 2型糖尿病人 (病例组 )和 130例正常人 (对照组 ) ,用PCR RFLP法分析每一受试对象的IL 6基因启动子区 174位点的基因型 ,并对每一受试对象进行口服 75g葡萄糖的糖耐量试验 ,用Homa model公式计算胰岛素抵抗指数 (IR) ,并测定空腹血清IL 6、血脂、血纤维蛋白原的含量 :将病例组和对照组按基因型不同分组 ,比较不同基因型的各组间的IL 6含量及IR ;多元回归法分析IL 6与IR、血脂、纤维蛋白原的相关性。结果 与对照组相比 ,病例组的IL 6的含量及IR显著升高 ;病例组 ,CC型的IL 6含量和IR最低 ,GC型次之 ,GG型最高 ,3型差异有显著性 (P <0 .0 5 )。在对照组 ,IL 6含量与IR的差异亦呈现类似趋势 (即CC型 <GC型 <GG型 ) ,但差异无显著性 (P >0 .0 5 )。多元回归分析显示病例组的IL 6亦与IR、纤维蛋白原、TG、HbA1C成正相关 ,与LDC C成负相关 ;对照组的IL 6与IR、HbA1C成正相关。结论 IL 6基因启动子区 174G/C多态性与IL 6的表达效率有关 ,该位点G等位基因与IL 6高表达有关 ,纯合子为GG型的个体IL 6水平及IR最高。 Objective To study the relationship between interleukin-6 gene-174G/C polymorphism and serum IL-6, insulin resistance in Type 2 diabetes.Methods 196 type 2 diabetes and 130 Controls were involved in the study.IL-6,gene type was analysed by PCR-RFLP.Glucose tolerance test was used and insulin resistance(INS-R) value was assessed by homeostasis mode assessment (HOMA).Serum IL-6,blood lipoid and fibronogen were also measured in fasting state.The correlations between IL-6 and INS-R,blood lipoid and fibronogen were determined by multiple linear regression analysis.Results Serum IL-6 and insulin resistance are significantly higher in diabetes than that in controls.In diabetes group,Compared with GC and GG,the CC gene type is associated with the lowest IL-6 levels and INS-R and the GG genetype has the highest IL-6 INS-R;And in controls,IL-6 and INS-R don't differ by genotype.In diabetes group,the serum IL-6 levels correlates positively with INS-R,fibronogen,and triglycerides and HbA 1C(r=0.47,0.34,0.58,0.45 respectively;P<0.05),and negatively with high-density lipoprotein-cholesterol(r=-0.39,P<0.05).In controls,the serum IL-6 levels correlates positively with INS-R,HbA 1C(r=0.29,0.37, respectively;P<0.05).Conclusion Polymorphism of IL-6 gene promoter-174G/C gene is associated with the altered gene expression and the G allele is assciated with higher levels of serum IL-6 and higher INS-R.;
