He-Ne激光血管内照射治疗椎基底动脉供血不足的BAEP观察

摘要 目的 :观察He Ne激光血管内照射 (ILIB)治疗椎基底供血不足 (VBI)的血液流变学及脑干听觉诱发电位 (BAEP)的变化 ,从电生理角度探讨ILIB治疗VBI的作用机理。方法 :将 6 4例VBI患者分为激光组 34例 ,采用He Ne激光治疗 ;常规组 30例 ,按神经内科常规治疗。 2组治疗前及治疗 1个月后检测血液流变学指标及BAEP ,并进行比较 ,观察He Ne激光对VBI患者血液流变学及脑电生理的影响。结果 :①治疗前患者血液粘度增高 ,表现为全血粘度高低切变值、血浆粘度及血脂增高。BAEP总异常率为 78.1% ,以脑干型异常为主。②治疗后 2组血液流变学及神经传导功能均有明显改善 (P <0 .0 5 )。与常规组比较 ,激光组血浆粘度及血脂水平降低更显著 (P <0 .0 5 ) ,BAEP中以V波PL、Ⅲ Ⅴ和Ⅰ ⅤIPL改善明显 (P <0 .0 5 )。结论 :He Ne激光血管内照射可改善VBI患者血液流变学情况及脑神经传导功能。 Objective: To observe the effect of He Ne laser intravascular irradiation therapy on the hemorrheology and brainstem auditory evoked potentials (BAEP) in vertebrobasilar insufficiency (VBI) and to explore the mechanism. Methods: Sixty four cases of VBI were divided into laser therapy group ( n =34) and routine therapy group ( n =30). The hemorrheology and BAEP were measured before and 1 month after treatment and compared. The effect of He Ne laser intravascular irradiation therapy on the hemorrheology and cerebral electrophysiology was observed. Results: (1) The viscosity of patients with VBI was increased before treatment. The whole blood viscosity shear value, plasma viscosity and blood fat were increased. The total abnormal rate of BAEP was 78.1% . The main abnormity was brainstem type; (2) The hemorrheology and the function of nerve conduction were improved distinctly ( P < 0.05 ) after treatment. Compared with the routine therapy group, the levels of plasma viscosity and blood fat were decreased markedly ( P < 0.05 ) in the laser therapy group, and PL of Ⅴ wave, Ⅲ Ⅴ and Ⅰ Ⅴ IPL were also improved significantly ( P < 0.05 ). Conclusion: The He Ne laser intravascular irradiation therapy can improve the hemorrheology and the function of nerve conduction in patients with VBI.
