微血管计数和血管内皮生长因子在胃癌浸润转移中的作用

Effects of Angiogenesis and Vascular Endothelial Growth Factor Expression on Invasion and Metastasis of Gastric Carcinoma

目的 研究微血管计数 (MVC)和血管内皮生长因子在胃癌组织中的表达情况 ,探讨其与临床病理特征的关系及在胃癌浸润转移中的作用。方法 应用免疫组织化学ABC法 ,检测 11例正常胃黏膜及 2 5 3例胃癌组织中MVC、血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子 (bFGF)的表达。结果 肿瘤组织MVC内明显高于正常胃黏膜组织 (P <0 .0 1) ;肿瘤组织中VEGF和bFGF阳性率分别为 67.6% ( 171/2 5 3)和 9.5 % ( 2 4/2 5 3)。VEGF的表达与分化程度、TNM分期、转移 (淋巴结、腹膜、肝等 )密切相关 (P <0 .0 1)。胃癌组织VEGF阳性组的MVC明显高于阴性组 (P <0 .0 1) ;VEGF表达阳性者复发转移率明显高于VEGF阴性者 (P <0 .0 1) ,VEGF阳性患者的预后明显较阴性者差。结论 高MVC及VEGF表达是胃癌的独立预后因子。

Objective To evaluate the effects of angiogenesis and vascular endothelial growth factor expression on invasion and metastasis of gastric carcinoma and their relationships with the clinicopathologic features.Methods The expressions of VEGF and basic fibroblast growth factor(bFGF) and microvessel count(MVC) were detected by immunohistochemical techniques(ABC) in 11 cases of normal gastric mucous and 253 cases of gastric carcinoma.Results The MVC in gastric carcinomas was higher than in normal gastric mucous( P <0.01).The positive rates of VEGF and bFGF in gastric carcinomas was 67.6%(171/253)and 9.45% (24/253)respectively.The expression of VEGF was well correlated with differentiation,TNM staging and metastasis(lymph node,peritoneum,liver etc. P <0.01).A close correlation was present between positive VEGF and MVC ( P <0.01).Moreover,the patients with positive VEGF gastric carcinoma had a significantly higher recurrence rate and poorer prognosis than negative ones.Conclusion The expressions of VEGF and MVC are independent prognostic factors in gastric carcinoma.VEGF may be an important factor responsible for the induction of angiogenesis in infiltration and metastasis of gastric carcinomas.
