原发性肝癌患者血清可溶性细胞间粘附分子-1的测定及其临床意义

Clinical Significance of Serum Intercellular Adhesion Molecule-1 in Patients with Primary Liver Cancer

目的 探讨血清可溶性细胞间粘附分子 1(sICAM 1)与原发性肝癌发展、转移、疗效及预后的关系。方法 采用ELISA法检测 72例原发性肝癌患者治疗前后、12例良性肝肿瘤患者治疗前血清中sICAM 1的水平 ,再进行比较分析。结果 原发性肝癌患者血清sICAM 1水平高于正常对照组及良性肝肿瘤患者 (P <0 .0 1) ,且与肿瘤直径、有无腹腔积液、有无肝外转移、临床分期及治疗效果有关。肿瘤越大 ,血清sICAM 1水平越高 (P <0 .0 1) ,有腹腔积液者比无腹腔积液者高 (P <0 .0 5 ) ,有肝外转移者比无肝外转移者高 (P <0 .0 5 ) ,Ⅲ期患者比Ⅱ期者高 ,Ⅱ期者又比Ⅰ期者高 (P <0 .0 5或P <0 .0 1) ,治疗前比治疗后高 (P <0 .0 1) ;而正常对照组与良性肝肿瘤患者血清sICAM 1水平无显著性差异 (P >0 .0 5 )。结论 原发性肝癌患者血清sICAM 1水平在一定程度上可以反映肝癌发展、转移情况及治疗效果 ,有可能成为预测肝癌转移。

Objective To study the relationship between serum soluble intercellular adhesion molecule 1(sICAM 1) and the development,metastasis,curative effect and prognosis of primary liver cancer.Methods The serum sICAM 1 level in 72 patients with primary liver cancer and 12 patients with various benign liver tumour were determined by ELISA method before and after treatment.Results The sICAM 1 level in serum of patients with primary liver cancer both prior to and after treatment was significantly higher than that in control group and the patients with benign liver tumour( P <0.01),and correlated with tumour diameter,ascites,metastasis,clinical stages and curative effect.The bigger the tumour diameter the higher the serum sICAM 1 level was( P <0.01),The sICAM 1 level in patients with ascites was higher than that in patients without ascites( P <0.05),The seurm sICAM 1 level in patients with metastasis was higier than that in patients without metastasis( P <0.05),The serum sICAM 1 level of patients in stage Ⅲ was significantly higher than that in stage Ⅱand Ⅰ,and it was higher in stage Ⅱ than in stage Ⅰ( P <0.05 or 0.01),There was a significant difference in the sICAM 1 level between pre and post treatment( P <0.01).But there was not singnificant difference in the serum sICAM 1 level between control group and the patients with benign liver tumour( P >0.05).Conclusion The serum sICAM 1 level could be a marker of the stage and metastasis of primary liver cancer and may play a role in the biological character of hepatoma cell and curative effect.
