阴囊Paget病的临床和病理研究

Clinicopathologic study of Paget's disease of scrotum

目的 探讨阴囊Ｐａｇｅｔ病的临床特点和诊治方法。方法 对１４例阴囊Ｐａｇｅｔ病患者的临床、病理资料以及治疗和随访情况进行回顾性研究。结果 １４例患者均经病理检查证实并行手术治疗，随访期２～１２年，ｌ例腹股沟淋巴结阳性者１年后死于全身转移，３例死于非癌疾病，３例行腹股沟淋巴结请扫者２例发生皮肤坏死、淋巴瘩等并发症。结论 本病的诊断主要依靠病理学检查，阴囊局部扩大切除术是首选治疗方法，预后主要取决于病变浸润和淋巴结转移的程度以及邻近内脏器官癌肿的有无，早期诊断是提高生存率的关键。

Objective To study the Clinicopathologic features and the treatment of Paget's disease involving the scrotum. Methods 14 cases of scrotal Paget's disease were studied retrospectively from 1978 to 1999. Results 14 cases of Paget's disease of scrotum were successfully treated surgically and diagnosis was confirmed on pathological studies. The follow up showed that 1 case of positive inguinal lymph nodes had died of whole metastasis after 1 year and 3 cases had died of other diseases. The lymphatic fistula and necrosis of skin were main complication. Conclutions Early biopsy and wide excision of the scrotal leison is the first choice of treatment for Paget's disease.
