腮腺手术应用胸锁乳突肌瓣转移预防味觉出汗综合征的效果观察

Application of sternocleidomastoid muscle flap in the prevention of Frey's syndrome following parotidectomy

目的 :了解腮腺手术采用胸锁乳突肌瓣转移对减少味觉出汗综合征发生率的作用。方法 :33例腮腺肿瘤患者行腮腺肿块及部分浅叶切除术。其中 15例同期行胸锁乳突肌瓣转移 ,另 18例按常规方法 ,均按期随访 ,且做碘 -淀粉试验检查作客观评价。结果 :经术后 8～ 18个月观察 ,胸锁乳突肌瓣转移组的 15例中碘 -淀汾试验 2例阳性 ,占 13 33% ;常规组 18例中9例阳性 ,占 5 0 % ,两者间存在显著差异 (P <0 0 5 )。结论 :腮腺部分切除术后行胸锁乳突肌瓣转移可明显降低味觉出汗综合征的发生率 ,是有效预防味觉出汗综合征发生的一种简单、实用 ,无严重并发症 。

s Objective:To explore the influence of sternocleidomastoid muscle flap covering the area of part parotidectomy on incidence rate of Freys syndrome.Methods:In this study 33 patients of parotidean tumor underwent the part superfacial lobectomy,15 patients of whom with performing the sternocleidomastoid muscle flap to cover the surface after part lobectomy(A group),the others without this flap(B group).Each patient was examined by iodine starch test with spreading out the skin of parotid gland region after operation (8～18 months).Results:Two patients were positive in A group and 9 patients were positive in B group.The incidence rate were significantly lower in A group than that in B group(χ 2=4 95?P<0 05).Conclusion:As stermocleidomastoid muscle flap is effective to lower incidence rate of Freys syndrome and to correct the sinking deformity after park lobectomy without serious complications,it is a simple and useful surgical method for this syndrome.
