高频彩色多普勒超声在乳腺肿瘤疾病中的诊断价值 被引量:32

摘要 目的探讨高频彩色多普勒超声在乳腺肿瘤疾病中的诊断价值。方法选择2016年1月至2018年1月铜川市耀州区人民医院收治的80例乳腺肿瘤患者为研究对象,其中以40例乳腺癌患者为观察组,40例乳腺良性肿瘤患者为对照组。所有患者均接受高频超声检查,比较乳腺癌与乳腺良性肿瘤高频超声特征、多普勒血流特征及BI-RADS分级情况,分析高频彩色多普勒超声对乳腺肿瘤的诊断效力。结果乳腺癌和乳腺良性肿瘤的纵横比≥1 (60.0%vs 35.0%)、存在微钙化(67.5%vs 37.5%)、边缘不规则(57.5%vs 27.5%)、Ⅲ级血流(50.0%vs 12.5%)的比例比较,乳腺癌均明显高于乳腺良性肿瘤,差异均有统计学意义(P<0.05);乳腺癌和乳腺良性肿瘤的BI-RADS分级4b (30.0%vs 7.5%)、4c (30.0%vs 5.0%)及5级(12.5%vs 2.5%)的比例比较,乳腺癌明显高于乳腺良性肿瘤,BI-RADS分级2级(0 vs 25.0%)、3级(7.5%vs 50.0%)的比例比较,乳腺癌明显低于乳腺良性肿瘤,差异均具有统计学意义(P<0.05);高频超声共将4例乳腺癌误诊为乳腺良性肿瘤,将8例乳腺良性肿瘤误诊为恶性肿瘤,诊断敏感度、特异度及准确度分别为90.0%、80.0%和85.0%。结论在乳腺肿瘤疾病的诊断和鉴别诊断中,高频彩色多普勒超声具有较高的诊断效力。 Objective To explore the diagnostic value of high frequency color Doppler ultrasound in breast tumor. Methods A total of 80 patients with breast tumors admitted to Tongchuan Yaozhou District People’s Hospital from January 2016 to January 2018 were selected as the study subjects. Among them, 40 patients with breast cancer were included in the observation group and 40 patients with benign breast tumors were selected as the control group. All patients underwent high-frequency ultrasonography. High-frequency ultrasonographic features, Doppler flow characteristics and BI-RADS classification of breast cancer and benign breast tumors were compared. The diagnostic effect of high-frequency color Doppler ultrasonography on breast tumors was analyzed. Results The ratios of aspect ratio ≥1 of breast cancer to benign breast tumors(60.0% vs 35.0%), microcalcification(67.5% vs 37.5%), irregular margin(57.5%vs 27.5%) and grade Ⅲ blood flow(50.0% vs 12.5%) were compared, the ratios of breast cancer were significantly higher than those of benign breast tumors(P<0.05). The proportions of BI-RADS grade 4 b(30.0% vs 7.5%), 4 c(30.0% vs5.0%) and grade 5(12.5% vs 2.5%) in breast cancer were significantly higher than those in benign breast tumors, while the proportions of BI-RADS grade 2(0 vs 25.0%) and grade 3(7.5% vs 50.0%) of breast cancer were significantly lower than those of benign breast tumors(P<0.05). Four cases of breast cancer were misdiagnosed as benign breast tumors and eight cases of benign breast tumors were misdiagnosed as malignant tumors by high-frequency ultrasound. The diagnostic sensitivity, specificity and accuracy were 90.0%, 80.0% and 85.0%, respectively. Conclusion High frequency color Doppler ultrasonography has high diagnostic efficacy in the diagnosis and differential diagnosis of breast tumors.
