基于miR-155/NF-κB信号通路探讨新风胶囊改善类风湿关节炎血瘀证患者肺功能的机制 被引量:20

摘要 目的:基于miR-155/NF-κB信号通路探讨类风湿关节炎(RA)患者血瘀状态与肺功能的关系及新风胶囊(XFC)对其影响。方法:选取RA患者60例,随机数字表法分为新风胶囊治疗组30例,来氟米特(对照)组30例。观察新风胶囊对RA患者细胞因子、凝血指标、NF-κB、miR-155、血瘀症状体征积分及肺功能参数的影响。结果:与轻度血瘀证组比较,XFC重度血瘀证组的FEV1/FVC、FEF50、FEF75显著降低(P<0.05,P<0.01),p65、miR-155显著升高(P<0.05,P<0.01)。与LEF重度血瘀证组比较,XFC重度血瘀证组FEV1/FVC、FEF50显著升高(P<0.01),p65、IL-6显著降低(P<0.01)。与治疗前比较,XFC组FEF50、FEF75、PEF、IL-10、IL-4、PAFAH显著升高(P<0.05,P<0.01),血瘀症状体征积分、IL-17、IL-6、D-D、FBG、 PLT、PAF、NF-κB通路指标、miR-155显著下降(P<0.05,P<0.01);与LEF组比较,XFC组的FEF50、PEF显著升高(P<0.05),关节刺痛、舌质、皮下瘀斑、血瘀总积分、IL-17、D-D、FBG、PLT、PAF、p65、p50显著下降(P<0.05,P<0.01)。且RA患者肺功能与细胞因子、血瘀指标、miR-155、NF-κB具有相关性。结论:RA血瘀证患者肺功能异常,可能与其体内miR-155的高表达、NF-κB的异常活化、凝血指标异常以及细胞因子网络的失衡有关,而新风胶囊能够通过调节miR-155/NF-κB信号通路,改善血瘀状态从而改善RA患者肺功能。 Objective:Based on miR-155/NF-κB signaling pathway,the relationship between blood stasis and pulmonary function in patients with rheumatoid arthritis and.the effect of Xinfeng Capsule on it were explored.Methods:Sixty patients with RA were enrolled.They were divided into 30 cases of Xinfeng Capsule group and 30 cases of leflunoraide group according to the cohort study method.To observe the effects of Xinfeng Capsule on cytokines,coagulation parameters,NF-κB,miR-155, blood stasis symptoms and lung function parameters of RA patients.Results:Compared with the mild blood stasis syndrome group,the FEV1/FVC,FEF50,and FEF75 in the XFC severe blood stasis syndrome group were significantly decreased (P<0.05, P<0.01),and p65 and miR-155 were significantly increased (P<0.05,P<0.01).Compared with LEF severe blood stasis syndrome group,FEV1/FVC and FEF50 were significantly increased in XFC severe blood stasis syndrome group (P<0.01),and p65 and IL-6 were significantly decreased (P<0.01).Compared with pre-treatment condition,FEF50,FEF75,PEF,IL-10,IL-4,PAF-AH in XFC group were significantly increased (P<0.05,P<0.01),blood stasis symptoms and signs,IL-17,IL-6,DD,FBG,PLT,PAF NF-κB pathway index and miR-155 decreased significantly (P<0.05,P<0.01).Compared with LEF group,FEF50 and PEF were significantly increased in XFC group (P<0.05),joint tingling,tongue quality,subcutaneous ecchymosis,total blood stasis score, IL-17,DD,FBG,PLT,PAF,p65,p50 decreased significantly (P<0.05,P<0.01).The lung function of RA patients was correlated with cytokines,blood stasis index,miR-155 and NF-κB.Conclusion:Abnormal lung function in patients with RA blood stasis syndrome may be related to the high expression of miR-155,abnormal activation of NF-κB,abnormal blood coagulation index and imbalance of cytokine network.Xinfeng Capsule can improve the pulmonary function of patients with RA by regulating miR-155/ NF-κB signaling pathway.
