电针治疗脑卒中后神经源性膀胱临床观察

Clinical observation of electro-acupuncture for treatment of neurogenic bladder after stroke

目的:基于临床基础药物治疗,观察电针与常规针法治疗脑卒中后神经源性膀胱的临床疗效差异。方法:符合纳入标准的60例脑卒中后神经源性膀胱患者随机分为电针组(30例)和对照组(30例)。二组均予醒脑开窍针刺法,电针组在此之上另取中极、关元、水道、大赫穴行电针治疗。比较两组临床疗效、排尿功能评价(I-PSS、ICI-Q-SF)、膀胱剩余尿量、生活质量评估、神经功能及症状评估(NIHSS)。结果:治疗后,电针组有效率优于对照组(P<0.05);治疗2周及随访时电针组I-PSS、ICI-Q-SF评分、生活质量评分降低更显著(P<0.05);随访期电针组NHISS较对照组降低更显著(P<0.05);治疗后,组间比较膀胱剩余尿量差异无统计学意义。结论:电针在改善神经源性膀胱患者排尿功能、生活质量方面有显著疗效;对改善神经系统功能远期疗效更佳;在改善残余尿量方面效果不明显。

Objective:To observe the differences of clinical efficacy between the electro-acupuncture and acupuncture in treating neurogenic bladder after stroke based on clinical basic drug treatment.Methods:Sixty neurogenic bladder after stroke patients who was conformed to the inclusion criteria were randomly divided into the electro-acupuncture group (30 cases)and the control group (30 cases).Both groups were given the basic acupuncture method of inducing resuscitation,but electro-acupuncture groupcombined with electro-acupuncture therapy on the acupoint as Zhongji,Guangyuan,Shuidao and Dahe.Compared clinical efficacy,micturition function evaluation (I-PSS,ICI-Q-SF),residual bladder volume,life quality assessment,neurological function and symptom assessment (NIHSS)of two groups.Results:After treatment,the electro-acupuncture group was better than the control group in effective rate (P<0.05).Electro-acupuncture group's I-PSS,ICI-Q-SF score and life quality score decreased significantly (P<0.05).During the follow-up period,the NHISS of the electro-acupuncture group decreased more remarkably than control group (P<0.05).After treatment,there was no statistically significant difference in the residual bladder volume between the groups.Conclusion:Electro-acupuncture had significant effect in improving micturition function and quality of life,but not obvious in residual bladder volume.
