曼月乐联合妈富隆治疗围绝经期功血的临床疗效及对血清FSH、LH、E_2、P水平的影响

The Clinical Curative Effect of Mirena Combined with Marvelon on Perimenopausal Dysfunctional Uterine Bleeding and Their Influences on Serum Levels of FSH,LH,E_2 and P

目的探讨曼月乐联合妈富隆治疗围绝经期功能失调性子宫出血(简称功血)的临床疗效及对血清激素水平的影响。方法随机数字法将120例围绝经期功血患者分为观察组与对照组,各60例。观察组采取曼月乐联合妈富隆治疗,对照组给予妈富隆单药治疗,比较各组临床疗效、止血、治疗前后血清激素[促卵泡生成素(FSH)、黄体生成素(LH)、雌激素(E_2)、孕酮(P)]、子宫内膜厚度(ET)及血红蛋白(Hb)变化,并评价各组用药安全性。结果观察组治疗总有效率100.0%,显著高于对照组的90.0%(P<0.05);观察组阴道出血控制时间、完全止血时间均显著短于对照组(P<0.05);观察组治疗后血清FSH、LH、E_2、P水平、ET均显著低于对照组,Hb水平显著高于对照组,差异有统计学意义(P <0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论曼月乐联合妈富隆治疗围绝经期功血疗效明确,能明显改善血清激素水平,且安全性较高。

Objective To investigate the clinical curative effect of Mirena combined with Marvelon on perimenopausal dysfunctional uterine bleeding(DUB) and their influences on serum hormones levels. Methods120 patients with perimenopausal DUB were randomly divided into the observation group and the control group,with 60 cases in each. The observation group were treated with Mirena combined with Marvelon while the control group were treated with Marvelon alone. The clinical curative effect,hemostasis,changes in serum hormones [ follicle-stimulating hormone( FSH), luteinizing hormone( LH), estrogen( E2), progesterone(P) ],endometrial thickness(ET) and hemoglobin(Hb) before and after treatment were compared between these two groups, and the medication safety was evaluated. Results The total treatment effective rate of the observation group was significantly higher than that of the control group( 100. 0% vs 90. 0%)(P <0.05).The time of vaginal bleeding control and the complete hemostasis time in the observation group were significantly shorter than those in the control group(P<0.05). After treatment,serum levels of FSH,LH,E2 and P and ET in the observation group were significantly lower than those in the control group,while the level of Hb was significantly higher than that in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P >0.05). Conclusion Mirena combined with Marvelon is effective in the treatment of perimenopausal DUB. The regimen can significantly improve levels of serum hormones, with high safety.
