成人肱骨远端骨折不愈合的治疗策略及效果分析

Treatments of humeral distal end fracture nonunion and their effects in adults

成人肱骨远端骨折不愈合在临床上较常发生,包括患肢疼痛、关节不稳、无力及尺神经炎等,严重影响患者生活质量,因此对于肱骨远端骨折不愈合的治疗,仍然是临床医生的重要任务。目前治疗肱骨远端骨折不愈合的方案主要包括切开复位内固定及植骨技术,外固定架及骨搬运技术及肘关节置换。本文检索已发表关于成人肱骨远端骨折治疗文献,包括植骨术、骨搬运、外固定架技术、切开复位内固定、关节融合及关节置换等治疗方法,旨在分析各种治疗方法取得的效果,确定不同类型肱骨远端骨折不愈合的治疗策略。

Humeral distal end fracture nonunion of the adults is often found in the clinical practice,including pain in the affected limb,joint instablity,stengthless,and ulnar neuritis,which strongly impacts on the life quality of the patients.Therefore,the treatment of humeral distal end fracture nonunion is an important task of the cXinician.Recently,the main treatments of humeral distal end fracture nonunion include open reduction and internal fixation,bone grafting,external fixator,bone transport and total elbow arthroplasty.In this article we reviewed the published papers about the therapies of humeral distal end fracture nonunion in adults including allograft,distraction,external fixator,open reduction and internal fixation,bone transport,arthrodesis and arthroplasty,in order to make clear the treatment methods of humeral distal end fracture nonunion of dif- ferent types by analyzing the outcomes of different therapies.
