摘要
目的:研究多层螺旋CT(MSCT)三维重建对小儿气管、支气管异物的诊断价值。方法:选取2016年4月~2017年9月新郑市人民医院疑似气管、支气管异物患儿52例,均行MSCT三维重建检查,以气管镜探查术为"金标准",分析MSCT三维重建灵敏度、准确度。结果:本研究52例患儿中MSCT三维重建检查后均发现存在异物;对52例患儿行气管镜探查术检查,发现并成功取出异物患儿51例,其中1例未发现异物,清除气道中分泌物,术后给予黏液稀化剂、抗生素治疗症状好转; MSCT三维重建检测小儿气管、支气管异物的灵敏度为100%(51/51),准确度为98. 08%(51/52)。MSCT三维重建阳性率100%(52/52)与气管镜探查术98. 08%(51/52)对比,差异无统计学意义(P> 0. 05)。结论:MSCT三维重建检查小儿气管、支气管异物具有较高灵敏度、准确度,其检查结果同"金标准"相似,对临床具有重要诊断价值。
Objective: To study the diagnostic value of multislice spiral CT( MSCT) three-dimensional reconstruction in tracheal and bronchial foreign body of children. Methods: A total of 52 cases of suspected tracheal and bronchial foreign body in Xinzheng people’s Hospital from April 2016 to September 2017 were examined by MSCT three-dimensional reconstruction. Taking the tracheal microscopy as the " gold standard",the sensitivity and accuracy of the MSCT three-dimensional reconstruction were analyzed. Results: In this study,52 cases of children were found foreign bodies after MSCT three-dimensional reconstruction. Those 52 cases of children were examined by tracheoscopy,and 51 cases of foreign body were found successfully. One of them did not find foreign bodies and the secretions in the airway were removed. The symptoms of mucus dilute agents and antibiotics were improved after the operation. The symptoms were improved after given mucus dilute agents and antibioticsthe after operation. The sensitivity of MSCT three-dimensional reconstruction in tracheal and bronchial foreign body of children was 100%( 51/51),and the accuracy was 98. 08%( 51/52). There wasn’t any statistical difference in the positive rate between MSCT three-dimensional reconstruction 100%( 52/52) and the the tracheal microscopy 98. 08%( 51/52)( P > 0. 05). Conclusion: MSCT three-dimensional reconstruction in the diagnosis of children’s tracheal and bronchial foreign body has high sensitivity and accuracy. The results of the examination are similar to the " gold standard",and it has an important diagnostic value for the clinic.
