微弹簧圈行超选择性肾动脉栓塞术治疗MPCNL术后出血的疗效分析 被引量:3

摘要 目的:探讨微弹簧圈行超选择肾动脉栓塞术治疗MPCNL术后出血的疗效及并发症发生率。方法:选取我科采用微弹簧圈行超选择性肾动脉栓塞术治疗MPCNL术后出血患者30例,观察临床止血成功率、术中及术后并发症发生率。结果:30例患者均成功实施超选择性肾动脉栓塞术,技术成功率100%,28例患者一次成功止血,1例患者栓塞后24小时再次出血,量约500ml,再次行栓塞术,出血停止; 1例患者栓塞后出血未见缓解,第二天选择外科肾切除术,止血成功率96. 7%(29/30)。术后随访4~68个月,所有患者未出现肾脓肿、肾功能恶化、肾衰、异位栓塞等严重并发症。结论:采用微弹簧圈行超选择性肾动脉栓塞治疗MPCNL术后出血,止血效果确切、并发症少、对肾功能影响小,值得临床广泛应用。 Objective: To explore the clinical value and complication of super selective renal artery embolization with coil for the treatment of hemorrhage after MPCNL Method Selected 30 cases of hemorrhage underwent super selective renal artery embolization after MPCNL admitted to our department,the clinical efficiency of hemostasis and the incidence of postoperative complications were analyzed. Results: Operations were successful 30 cases,with the operation success rate of100%,hemostasis were successful in 28 patients of the 30 cases of patients one time,1 cases which hemorrhage excess500 ml within 24 hours after embolization which was stopped after a second embolization,1 cases which hemostasis no remission after embolization,surgical nephrectomy after 24 hours,the success rate of hemostasis is 96. 7%( 29/30). 30 cases were followed up 4 to 68 months,which not nounderwent severe complications,such as renal abscess,renal function,renal failure,ectopic embolism. Conclusion: Coil Application to superselective renal artey embolization in the treatment of hemorrhage after MPCNL has advantages of good curative effect,little trauma,less complications and less influence on renal function,is worthy of widely clinical application.
