摘要
目的:研究探讨多层计算机断层扫描(multisice computed tomography,MSCT)灌注成像对孤立性肺结节的诊断价值。方法:病例收集时间:2014年2月~2018年5月;研究对象:选择我院就诊的58例孤立性肺结节患者。采用多层螺旋CT机,对这58例孤立性肺结节患者实施MSCT平扫、MSCT灌注成像,以手术病理诊断结果为参照,计算和比较MSCT平扫、MSCT灌注成像对肺癌的诊断灵敏度、特异度、准确率。根据手术病理诊断结果,将孤立性肺结节患者分为肺癌组、良性病变组,比较两组的MSCT灌注成像参数。结果:以手术病理诊断结果为参照,MSCT灌注成像对肺癌的诊断灵敏度、特异度、准确率均高于MSCT平扫(P <0. 05)。肺癌组患者的血容量、血流灌注量、强化峰值均高于良性病变组(P <0. 05),其到达峰值时间晚于良性病变组(P <0. 05)。结论:MSCT灌注成像对孤立性肺结节的诊断价值良好,可对肺结节性质予以鉴别区分。
Objective: To study the diagnostic value of multi-slice computed tomography( MSCT) perfusion imaging in solitary pulmonary nodules. Methods: Cases were collected from February 2014 to May 2018. Subjects: 58 patients with solitary pulmonary nodules were selected. MSCT plain scan and MSCT perfusion imaging were performed in 58 patients with solitary pulmonary nodules. The sensitivity,specificity and accuracy of MSCT plain scan and MSCT perfusion imaging in the diagnosis of lung cancer were calculated and compared with the results of pathological diagnosis. According to the results of surgical and pathological diagnosis,patients with solitary pulmonary nodules were divided into lung cancer group and benign lesion group,and MSCT perfusion imaging parameters of the two groups were compared. Results: The sensitivity,specificity and accuracy of MSCT perfusion imaging in the diagnosis of lung cancer were higher than those of MSCT plain scan( P < 0. 05). The blood volume,blood perfusion and enhancement peak value of lung cancer group were higher than those of benign lesion group( P < 0. 05),and the peak value time was later than that of benign lesion group( P < 0. 05). Conclusion: MSCT perfusion imaging is of good value in the diagnosis of solitary pulmonary nodules and can be used to differentiate the nature of pulmonary nodules.
