晶体和胶体溶液预扩容对腰-硬联合麻醉下围术期患者血流动力学的影响

Effect of Pre-dilation of Crystal and Colloidal Solution on Hemoglobin Changes in Perioperative Patients with Loin-hard combined Anesthesia

目的探讨晶体和胶体溶液预扩容对腰-硬联合麻醉下围术期患者血流动力学的影响。方法选择我院2015年1月-2018年1月腰-硬联合麻醉下择期手术患者作为研究对象,一共90例,根据容量优化输入液体的类型进行分组,胶体溶液组采取胶体液6%羟乙基淀粉(HES)1000mL完成预充,晶体溶液组采取晶体液乳酸林格氏液(RL)1000mL完成预充,比较两组患者的血流动力学指标水平。结果与预充前对比,晶体溶液组预充后Hb、TFC明显增加,麻醉后SBP、SVR明显降低,P<0.05;胶体溶液组预充后SBP、Hb、CO明显降低,TFC明显增加,P<0.05;与晶体溶液组对比,胶体溶液组预充后SBP、Hb、CO明显更低,TFC明显更高,P <0.05。结论在腰-硬联合麻醉下围术期患者预扩容中,胶体溶液容量扩容效果也明显优于晶体溶液。

Objective To investigate the effects of predilation of crystal and colloidal solution on hemoglobin changes in perioperative patients with loin-hard combined anesthesia.Methods To chose patients with selective surgery from January 2015 to January 2018 under the combined waist and hard anesthesia.A total of 90 cases were grouped according to the type of liquid input with optimum capacity.The colloidal solution group took 6% of the body fluids.Hydroxy ethyl starch(HES)1000mL completed precharging.In the crystal solution group,the pre-charge of the liquid Ringer's solution(RL)1500mL was completed and the hemodynamics index levels of the two groups were compared.Results Compared with pre-charge,Hb and TFC increased obviously after pre-charge in crystal solution group,SBP and SVR decreased obviously after anesthesia,P<0.05;Compared with crystal solution group,SBP,Hb,CO were significantly lower,TFC was significantly higher,P<0.05.Conclusion The effect of colloidal solution capacity expansion'was better than that of crystal solution in the predilation of perioperative patients.
