慢性阻塞性肺疾病(COPD)急性加重期患者应用布地奈德联合异丙托溴铵雾化吸入治疗的临床效果观察 被引量:6

摘要 目的观察采取布地奈德联合异丙托溴铵雾化吸入治疗慢性阻塞性肺疾病(COPD)急性加重期患者的临床效果分析。方法将在我院呼吸科就诊,近期(2016年03月-2018年03月时期)收治以急性加重期COPD患者总计78例,随机数字法分成单用组(39例)与联合组(39例)。两组患者均予常规治疗,单用组配合异丙托溴铵雾化吸入治疗,联合组在对照组基础上联合布地奈德雾化吸入治疗。对照两组患者临床治疗效果、不良反应率,治疗前、后血气与肺功能各项指标。结果联合组患者临床治疗总有效率为94.87%,单用组为79.49%,差异具有统计学意义(P<0.05)。联合组患者不良反应率为2.56%,单用组为7.69%,差异有统计学意义(P<0.05)。联合组患者血气、肺功能各项指标经治后优于单用组,差异具有统计学意义(P<0..001)。结论布地奈德联合异丙托溴铵雾化吸入治疗COPD急性加重期患者,安全有效,明显改善治疗后血气与肺部各项功能指标,效果理想。 Objective To observe the clinical effect of budesonide combined with compound ipratropium bromideinhalation in the treatment of chronic obstructive pulmonary disease (COPD) in acute phase. Methods 78 patients withacute COPD were admitted to the respiratory department of our hospital from March 2016 to March 2018. They wererandomLy divided into single-use group (39 cases) and combined group (39 cases). Both groups were given routinetreatment such as resolving phlegm, relieving asthma, relieving cough, reducing inflammation and low flow oxygeninhalation. The single group was treated with compound ipratropium bromide aerosol inhalation, and the combinedgroup was treated with Budesonide aerosol inhalation on the basis of the control group. The clinical therapeutic effect,adverse reaction rate, blood gas and lung function before and after treatment were compared between the two groups.Results The total effective rate was 94.87% in the combined group and 79.49% in the single group (P<0.05). The adversereaction rate of the combined group was 2.56% and that of the single group was 7.69%. There was significant differencebetween the two groups (P<0.05). There was no significant difference in the indexes of blood gas and lung functionbetween the two groups before treatment (P>0.05). After treatment, the indexes of blood gas and lung function in thecombined group were better than those in the single group, and the difference was statistically significant (P<0.001).Conclusion Budesonide combined with compound ipratropium bromide aerosol inhalation is safe and effective in thetreatment of patients with acute COPD. It can obviously improve the blood gas and lung function after treatment.
