复原益肾活血汤联合黄体酮胶囊治疗人工流产后月经不调的临床研究

Clinical Study on Irregular Menstruation after Artificial Abortion by Fuyuan Yishen Huoxue Decoction(复原益肾活血汤) Combined with Progesterone Capsule

目的:观察复原益肾活血汤联合黄体酮胶囊治疗人工流产后月经不调的临床效果。方法:选取内蒙古自治区中医医院收治的人工流产后月经不调患者114例为研究对象,将其随机分为对照组与治疗组,各57例,对照组给予黄体酮胶囊治疗,治疗组给予复原益肾活血汤联合黄体酮胶囊治疗,两组均持续治疗3个月经周期。观察两组治疗前后中医证候积分、子宫内膜厚度、行经时间、月经血量、子宫螺旋动脉血流参数、生活质量调查表(SF-36)评分,比较两组临床疗效及安全性。结果:治疗后两组中医证候积分较治疗前显著降低(P<0.05),且治疗组显著低于对照组(P<0.05);治疗后两组子宫内膜厚度、行经时间、月经失血图(PBAC)评分均较治疗前显著增加(P<0.05),且治疗组显著优于对照组(P<0.05);治疗后两组子宫螺旋动脉收缩期峰值流速/舒张末期流速(S/D)较治疗前显著升高、阻力指数(RI)和搏动指数(PI)较治疗前显著降低(P<0.05),且治疗组显著优于对照组(P<0.05);治疗组总有效率为98.25%,显著高于对照组的85.96%(P<0.05);治疗后两组SF-36评分均较治疗前显著提高(P<0.05),且治疗组显著优于对照组(P<0.05);两组不良反应比较,差异无统计学意义(P>0.05)。结论:复原益肾活血汤联合黄体酮胶囊治疗人工流产后月经不调患者,可显著改善患者临床症状,提高生活质量,疗效肯定。

Objective:To investigate the effect of Fuyuan Yishen Huoxue decoction combined with progesterone capsule on patients with irregular menstruation after artificial abortion.Methods:A total of 114 patients with irregular menstruation after artificial abortion treated in TCM Hospital in Inner Mongolia Autonomous Region were randomly divided into control and treatment groups,57cases in each group.Patients were given progesterone capsules in control group,and Fuyuan Yishen Huoxue decoction combined with progesterone capsule in treatment group.The patients were treated for 3menstrual cycles in the two groups. TCM symptom score,endometrial thickness,menstruation time,menstrual blood loss,uterine spiral artery blood flow parameters and the short from health survey(SF-36)scores were observed in the two groups efore and after treatment,and the clinical efficacy and safety were compared between the two groups .Results:After treatment, TCM symptom scores were significantly decreased in the two groups (P<0.05),and the treatment group showed lower TCM symptom score than the control group (P<0.05);After treatment,the endometrial thickness, menstruation time and pictorial blood loss assessment chart (PBAC)scores were significantly increased in the twogroups (P<0.05),and the treatment group showed better indexes than the control group (P<0.05);After treatment, the uterine spiral arteries peak systolic velocity/end-diastolic volume (S/D)were significantly increased and resistance index (RI)and pulsatility index (PI)were significantly decreased in the two groups;the treatment group showed better indexes than the control group (P<0.05).The total effective rate was 98.25%in treatment group and 85.96%in the control group;the treatment group showed higher total effective rate than control group (P<0.05);After treatment,the SF-36scores were significantly increased in the two groups (P<0.05),and the treatment group showed higher SF-36scores than the control group (P<0.05).During treatments,there was no statistically significant difference on adverse reactions between the two groups (P>0.05).Conclusion:Fuyuan Yishen Huoxue decoction combined with progesterone capsule can significantly relieve clinical symptoms in patients with irregular menstruation after artificial abortion,and improve the quality of life,with certain curative effect.
