《当代名老中医典型医案集》中脾胃虚类证的方药关联分析

Association Analysis on Prescriptions for the Spleen and Stomach Deficiency in Contemporary Old Chinese Typical Cases Set

目的:探讨《当代名老中医典型医案集》中治疗脾胃虚类证的方剂用药规律,以期为脾胃虚类证的临床治疗提供一定的参考。方法:收集《当代名老中医典型医案集》丛书中以脾胃虚类证候为主要证候的医案,对其中出现的方药进行"关联分析"。结果:药物的关联分析提炼出核心药组为党参、茯苓、白术、甘草。消痰的药对为陈皮和半夏,健脾的药对为茯苓和白术,健脾消食的药对为神曲和白术;药物和症状的关联提炼出经常便溏和倦怠乏力与茯苓强关联,胸闷和黄芪强关联,腹痛与芍药和甘草药对强关联。结论:《当代名老中医典型医案集》中补气健脾及消食化痰类方药在治疗脾胃虚类证中使用较多,符合临床实际。

Objective:To investigate the law of spleen and stomach syndrome deficiency prescription in Contemporary Old Chinese Typical Cases Set,in order to provide a reference for clinical treatment of spleen and stomach deficiency syndrome.Methods:The Contemporary Old Chinese Typical Cases Set series was chosed as the data sources,and association analysis was carried out to analyze the prescriptions of the selected medical records.Results:The drug association analysis showed that core group included Dangshen (codonopsis),Fuling (poria),Baizhu (atractylodes)and Gancao (licorice).The core group for phlegm included Chenpi (tangerine peel) and Banxia (pinellia),and spleen herbs included Fuling (poria)and Baizhu (atractylodes).The core group for forti- fy the spleen and disperse food included Shenqu (medicated leaven)and Baizhu (atractylodes).Loose stools and fatigue were strongly related to Fuling (poria);chest tightness was strongly related to Huangqi (astragalus membranaceus);and abdominal pain was strongly related to Shaoyao (peony)and Gancao (licorice).Conclusion:In theContemporary Old Chinese Typical Cases Set,the prescriptions for tonifying Qi and invigorating the spleen and eliminating phlegm and removing phlegm are frequently used in the treatment of spleen and stomach deficiency syndrome,which is in line with clinical practice.
