加味桂枝芍药知母汤治疗寒湿入络型结节性红斑疗效观察

Observation on the Curative Effect of Supplemented Cinnamon Twig Decoction Plus Peony and Common Anemarrhena Rhizome in the Treatment of Erythema Nodosum of Cold-dampness Type

目的对比观察加味桂枝芍药知母汤治疗寒湿入络型结节性红斑的临床疗效。方法将2016年11月—2017年12月黑龙江中医药大学附属第二医院皮肤科收治的70例寒湿入络型结节性红斑患者按照随机数表法随机分为治疗组(35例)与对照组(35例),其中治疗组患者采用加味桂枝芍药知母汤治疗,对照组患者采用雷公藤多苷片治疗,对比观察治疗4周后两组患者的皮损评分及临床·疗效。结果治疗4周后,治疗组患者的皮损评分为(2.83±2.66)分,对照组患者的皮损评分为(5.94±3.83)分,两组对比采用t′检验,t′=3.946, P<0. 01,差异具有统计学意义;治疗4周后,治疗组患者中痊愈14例、显效11例、有效8例、无效2例、总有效率为94.3%,对照组患者中痊愈7例、显效10例、有效8例、无效10例、总有效率为71.4%,两组对比采用等级资料的秩和检验,mann-whitney U=409. 000, Z=-2.480,P=0.013,P<0.05,差异具有统计学意义。结论加味桂枝芍药知母汤可有效缓解寒湿入络型结节性红斑的疼痛,促进红斑的消退,疗效显著,值得临床推广应用。

Objective To compare the clinical efficacy of Supplemented Cinnamon Twig Decoction Plus Peony and Common Anemarrhena Rhizome in the treatment of erythema nodosum of cold-dampness type. Methods Seventy patients with cold and dampness-type erythema nodosum, admitted to the Dermatology Department of the Second Affiliated Hospital of Heilongjiang University of Chinese Medicine from November 2016 to December 2017, were randomly divided into treatment group(35 cases) and control group(35 cases) according to the random number table. Patients in the treatment group were treated with Supplemented Cinnamon Twig Decoction Plus Peony and Common Anemarrhena Rhizome while patients in the control group with tripterygium glycosides tablets. Skin lesion score and clinical efficacy were observed and compared between the two groups after 4 weeks of treatment. Results After 4 weeks of treatment, skin lesion scores were(2. 83 ±2. 66) points and(5. 94 ±3. 83) points respectively in the treatment group and the control group, between which t’ test was used for comparison, and statistically significant difference was observed(t’ =3. 946. P < 0. 01); in the treatment group, 14 cases were cured, 11 cases showed marked effectiveness, 8 cases were effective, 2 cases were ineffective and the total effective rate was 94. 3%, while in the control group, 7 cases were cured, 10 cases showed marked effectiveness, 8 cases were effective, 10 cases were ineffective and the total effective rate was 71. 4%. The rank sum test for ranked data was used for the clinical efficacy comparison between the two groups and the results showed statistically significant difference( mann-whitney U = 409. 000, Z =-2. 480, P =0. 013. P < 0. 05). Conclusion Supplemented Cinnamon Twig Decoction Plus Peony and Common Anemarrhena Rhizome can effectively alleviate the pain caused by cold and dampness-type eiythema nodosum and accelerate the regression of eiythema, deserving to be promoted in clinical practice.
