2011—2016年山西省HIV/AIDS病例晚发现情况及影响因素

Late diagnosis of newly identified HIV/AIDS cases and its influencing factors in Shanxi,2011-2016

目的分析2011-2016年山西省新报告艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人(简称HIV/AIDS病人)晚发现情况及其影响因素。方法通过中国艾滋病综合防治信息系统,选择2011-2016年山西省新报告HIV/AIDS病例,按照国家晚发现通用标准,分析病例晚发现情况,并采用二分类Logistic回归分析其影响因素。结果 2011-2016年山西省新报告的5 213例HIV/AIDS病人中,晚发现比例为36.16%(1 885例)。多因素分析表明:晚发现比例是医疗机构来源病例[比值比(OR)=2.139,95%可信区间(CI):1.854~2.467]高于自愿咨询检测病例,异性性传播感染病例(OR=1.313,95%CI:1.139~1.513)高于同性性传播感染病例,50岁以上病例(OR=5.052,95%CI:3.422~7.459)高于20岁以下病例。结论山西省近几年报告HIV/AIDS病例数逐年增多,晚发现比例有所降低却未有效遏制,应进一步采取更有效的策略和措施及早发现艾滋病病例。

Objective To analyze the late diagnoses of newly identified HIV/AIDS cases and its influencing factors in Shanxi from 2011 to 2016.Methods Newly identified HIV/AIDS cases from 2011 to 2016 were selected from the national HIV/AIDS comprehensive information system.Rates related to LD were stratified according to the demographic indicators. The multivariate logistic regression methods were used to analyze the influencing factors of late diagnoses.Results A total of 5213HIV/AIDS cases were reported between 2011 and 2016,and the percentage of late diagnosis was 36.16%.Multivariate analysis indicated that the proportion of late diagnosis among the cases detected from medical institutions was higher than those among voluntary counseling and testing clinics (adjusted OR=2.139,95% CI:1.854-2.467).The proportion of late diagnosis among the cases from heterosexual transmission (OR:1.313,95% CI:1.139-1.513)was higher than that of homosexual transmission cases.Cases older than 50 had higher proportion of late diagnosis than cases under 20 years old (OR=5.052,95% CI:3.422-7.459).Conclusion Newly identified HIV/AIDS cases increase year by year in Shanxi.The late diagnosis of HIV/AIDS decrease but not effectively controlled.More effective strategies and measures are needed to promote the early diagnosis.
