摘要
目的探讨高频超声对乳腺癌HER-2表达的诊断价值。方法收集术后病理证实为乳腺癌患者93例,所有标本均常规行HER-2免疫组织化学检测,分析乳腺癌各超声声像图征象与HER-2阳性表达之间的相关性。结果确诊乳腺癌的93例患者中,HER-2阳性表达37例,HER-2阴性表达56例(39. 8%),超声参数:肿块最大径线≥2cm、微钙化、腋窝肿大淋巴结在乳腺癌HER-2表达阳性与阴性间有统计学意义(P <0. 05)。3项有统计学意义的指标联合分析,在HER-2阳性组中,符合0~3项特征者分别为1例(3. 4%)、5例(23. 8%)、18例(60%)、13例(100%)。结论高频超声声像图部分参数可在一定水平预测乳腺癌HER-2的表达,相较单项参数,不同参数联合分析可提高HER-2阳性乳腺癌的检出率,对HER-2阳性乳腺癌的诊断意义更大。
Objective To evaluate the diagnostic value of high-frequency ultrasound in the expression of HER-2in breast cancer.Methods Ninty-three cases of breast cancer confirmed by pathology after operation were collected.All specimens were routinely examined with HER-2immunohistochemistry,and the correlation between ultrasound signs and positive expression of HER-2was analyzed.Results In 93cases of breast cancer,37cases of positive expression of HER-2and 56cases of negative expression of HER-2were found,the positive rate was 39.8%.The ultrasonic parameters were as follows:the maximum diameter of mass ≥2cm,microcalcification and axillary lymphoid enlargement in the HER-2table of breast cancer.There was significant difference between positive and negative(P <0.05).In HER-2positive group,there were 1case with 3 items and 5cases with 3.8%of HER-2positive group,and 18cases(23.8%)were divided into two parts :60 cases (13cases)and 100cases (P <0.05),respectively,according to the three statistically significant indexes of HER-2positive group.Conclusion Some parameters of high frequency ultrasound image can predict the expression of HER-2in breast cancer at a certain level.Compared with single parameter,the combined analysis of different parameters can improve the detection rate of HER-2positive breast cancer, which is more important for the diagnosis of HER-2positive breast cancer.
