食管鳞状细胞癌组织中NF-κB,COX-2的表达及其临床意义

摘要 目的检测食管鳞状细胞癌组织及正常食管组织(距肿瘤部位5cm以上)核因子-κB(NF-κB)和环氧合酶-2(COX-2)的表达水平及与临床病理指标关系,探讨其在食管鳞状细胞癌发生发展中的作用及其相互关系。方法选取2011年1月~2013年1月在我院胸外科行食管癌根治术的食管鳞状细胞癌患者50例,选取同期在我院胸外科行食管癌根治术的10例正常食管黏膜标本作为对照,采用免疫组化SP法检测癌组织和正常食管组织中NF-κB,COX-2的表达水平,分析其在不同组织中的阳性表达率、与患者临床病理指标关系、相互关系及与食管鳞状细胞癌的关系。结果 (1)NF-κB在食管鳞状细胞癌的阳性率明显高于正常食管组织,两者比较,差异有显著性(P <0. 001),COX-2在食管鳞状细胞癌组织的阳性率明显高于正常食管组织,两者比较,差异有显著性(P <0. 001)。(2)NF-κB在食管鳞状细胞癌组织中的表达与患者的年龄、性别、部位、浸润深度、临床分期无关(P> 0. 05),而与食管鳞状细胞癌有无淋巴结转移、分化程度有关(P <0. 05)。COX-2在食管鳞状细胞癌组织中的表达与患者的年龄、性别、部位、浸润深度、临床分期、分化程度无关(P> 0. 05),而与淋巴结转移有关(P <0. 05)。(3)食管鳞状细胞癌组织中NF-κB与COX-2的表达呈正相关关系,相关系数γ=0. 5238(P <0. 01)。结论 NF-κB,COX-2在食管鳞状细胞癌发生发展中起重要作用,参与了食管鳞状细胞癌的发病,且与食管鳞状细胞癌的侵袭、转移及预后有一定的关系,对评价食管鳞状细胞癌的恶性程度、有无转移和预后有一定的参考价值。在食管鳞状细胞癌中,NF-κB与COX-2表达呈正相关,表明其在食管鳞状细胞癌的发生发展中起协同作用。 Objective To detect the expression levels of NF-κB and COX-2 in the tissue of esophageal squamous cell carcinoma and normal esophageal mucosa. To investigate the relationship of their expression with clinicopathologic factors and prognosis. To explore their correlation and their role in the development of esophageal squamous cell carcinoma. Methods Carcinoma tissues were obtained from 50 esophageal squamous cell carcinoma patients who had been surgically treated during Jan. 2011 to Dec. 2013 in Weifang Traditional Chinese Medicine Hospital. Normal tissues were obtained from 10 patients who had been checked with gastroscope biopsy at Jan. 2011 in Weifang Traditional Chinese Medicine Hospital. The expression levels of NF-κB,COX-2 in cancer tissues and normal esophageal tissues were detected by SP immunohistochemical method. The positive rates of expression of NF-κB,COX-2 in different tissues were analyzed. Results The expression of NF-κB and COX-2 in esophageal squamous cell carcinoma tissues was significantly higher than that in normal esophageal tissues. The NF-κB expression was irrelevant to age,gender,location,infiltration depth and clinical stage( P > 0. 05),it was associated with lymph node metastasis and differentiation( P < 0. 05). The COX-2 expression was irrelevant to age,gender,location,infiltration depth,clinical staging and differentiation( P > 0. 05),it was associated with lymph node metastasis( P < 0. 05). NF-κB expression was positive correlated with COX-2 in esophageal squamous cell carcinoma,coefficient correlation was 0. 5238( P < 0. 01). Conclusions NF-κB and COX-2 play an important part in carcinogenesis and development of esophageal squamous cell carcinoma. Besides,it is also associated with the invasion,metastasis and prognosis of esophageal squamous cell carcinoma. It is valuable to evaluate the degree of malignancy,metastasis and prognosis. NF-κB expression is positive correlated with COX-2 in esophageal squamous cell carcinoma and has synergistic effect on the carcinogenesis and development of esophageal squamous cell carcinoma.
