癫痫患者外周血淋巴细胞亚群变化的临床分析

Clinical analysis of the changes of peripheral blood lymphocyte subsets in patients with epilepsy

目的:从外周血淋巴细胞亚群水平变化来观察癫痫患者免疫功能情况,并进一步分析其与脑结构性改变之间是否存在相关性。方法:入组93例癫痫患者为研究组,60例健康正常人为对照组,采用流式细胞术检测其外周血淋巴细胞亚群水平,比较两组间结果,并结合头颅MRI结果进行分析。结果:①与健康对照组相比,研究组B细胞抗原CD19、T辅助细胞CD3+CD4+、NK细胞CD3-CD(16+56)+和CD4/CD8比值等指标显著升高,差异具有统计学意义(P<0.05);而总T淋巴细胞CD3+和T抑制细胞CD3+CD8+,未发现有统计学意义的差异(P>0.05)。②以MRI结果作为是否存在颅脑结构性改变的依据进行分析,未发现颅脑结构性改变与癫痫患者外周血淋巴细胞亚群水平之间存在有统计学意义的相关性(P>0.05)。结论:癫痫患者体液免疫功能和细胞免疫功能均较健康人群增强,免疫性病因在癫痫发病中具有重要地位,但和癫痫的结构性病因之间并不一定存在必然的联系。

Objective:To investigate the abnormality of immune function of patients with epilepsy based on the changes of peripheral blood lymphocyte subsets,and analyze the correlation between the changes of peripheral blood lymphocyte subsets and brain structural changes.Methods:93 patients with epilepsy were enrolled in the study group and 60 healthy people were served as controls.Flow cytometry (FCM)was applied to analyze the levels of peripheral blood lymphocyte subsets of both groups.MRI was also applied to detect the brain structural imaging of the two groups .Results:1. Compared with the healthy controls,the levels of B cell antigen CD19,T helper CD3+CD4+,NK ceil CD3-CD(16+56)+and CD4/CD8 ratios were significantly higher in the study group,.The difference was statistically significant .(P<0.05);whereas the levels of total T lymphocytes CD3+ and T inhibited CD3+CD8+showed no significant difference (P<0.05).2.Based on MRI results, there was no obvious correlation between brain structural changes and changes of peripheral blood lymphocyte subsets of patients with epilepsy (P>0.05).Conclusion:The humoral immune function and cellular immune function of patients with epilepsy are much enhanced compared with healthy people. The immune etiology plays an important role in epilepsy,but it is not necessarily related to the structural etiology of epilepsy.
